示踪剂联合化疗药物在进展期胃癌病例中双途径异时应用研究
Dual pathway of tracer combined chemotherapy in advanced gastric cancer
【摘要】 目的 :探讨采用纳米碳示踪剂联合化疗药物辅助胃癌根治手术进行淋巴结清除及术中、术后结合双途径化疗药物进行化疗治疗的临床效果。方法:选取本院胃肠外科拟行胃癌根治术D2式全胃切除术的80例患者采取随机数字表法分为研究组和对照组各43例,研究组术中采用纳米碳示踪联合化疗药物指导术中淋巴结清除,对照组行常规淋巴结清除,术后研究组患者采用腹腔灌注纳米碳联合化疗药物结合全身静脉化疗进行治疗,而对照组术后仅行全身静脉化疗,对比两组患者的临床效果。结果:研究组患者的淋巴结清除数目(23.6±6.2)枚、淋巴结转移阳性率(45.7±5.9)%,均显著高于对照组患者(18.2±5.4)枚、(33.8±6.2)%,且差异具有统计学意义(P<0.05);研究组和对照组的1、3、5年存活率组间比较差异均无统计学意义(P>0.05);研究组患者的中位生存时间46个月,显著长于对照组的41个月(χ2=4.018,P<0.05)。结论:纳米碳示踪剂联合化疗药物辅助胃癌根治手术有利于术中指导淋巴结清除和淋巴结靶向化疗,术后腹腔灌注纳米碳结合化疗药物效果较好,可能对患者远期生存时间有利。
【Abstract】 Objective: To investigate the clinical effect of combined chemotherapy with nano carbon tracer combined with chemotherapy in the treatment of lymph node dissection, intraoperative and postoperative chemotherapy drugs combined with double pathway chemotherapy. Methods:Selected hospital gastrointestinal surgery scheduled for radical gastrectomy D2 total gastrectomyformula of 80 patients taken randomly divided into study group and control group 43 cases, the study group used carbon nano tracer technique combined with chemotherapy drugs guidance intraoperative lymph node dissection, the control group underwent conventional lymphadenectomy and postoperative study group were treated with intraperitoneal chemotherapy combined with carbon nano joint intravenous chemotherapy treatment, while the control group only received postoperative intravenous chemotherapy, clinical results compared two groups of patients.Results: Cleaning the number of(23.6±6.2)medals, lymph node metastasis rate(45.7±5.9)% was significantly higher than patients(18.2±5.4)gold,(33.8±6.2)% of patients with lymph node study group and the difference was statistically significance(P<0.05);There was no significant difference in the survival rate between the study group and the control group(P>0.05)in 1, 3 and 5 years, and the median survival time of the study group was significantly longer than that of the control group for 41 months and the difference was statistically significant(χ2=4.018,P<0.05). Conclusion: Nano- carbon tracer gastric cancer in combination with chemotherapy drugs assisted surgery in favor of surgery and lymph node dissection guide targeted chemotherapy and postoperative intraperitoneal chemotherapy nano- carbon bond better, may be beneficial in patients with long- term survival time.
