【摘要】 目的观察吗啡联合地佐辛术后自控镇痛的效果。方法收集2014年8月-2016年2月骨科和肝胆外科择期大中手术患者120例,按随机数字表法随机分为3组,每组40例。术后采用经静脉途径患者自控镇痛,吗啡组镇痛药物为吗啡0.5mg/(kg·d),地佐辛组镇痛药物为地佐辛0.5mg/(kg·d),联合组为吗啡0.25mg/(kg·d)+地佐辛0.25mg/(kg·d)。观察术后6、12、24、48h静态和动态VAS评分,镇痛有效率,补救镇痛药物使用情况及不良反应发生情况。结果 3组术后6、12、24、48h静态VAS评分、动态VAS评分、镇痛有效率、补救镇痛药物使用情况差异均无统计学意义(P>0.05)。联合组不良反应发生率显著低于吗啡组、地佐辛组,差异均有统计学意义(P<0.05)。结论吗啡联合地佐辛用于大中手术后镇痛效果好,小剂量地佐辛可增强吗啡镇痛作用,进而减少吗啡用量,降低不良反应。更多还原

【Abstract】 Objective To observe the clinical effect of morphine combined with dezocine in postoperative analgesia.Methods 120 cases patients with surgery in orthopedics and hepatobiliary surgery from August 2014 to February 2016,were randomly divided into three groups,each of 40 cases. After surgery,adopted patient-controlled analgesia. The analgesic drugs of morphine group was morphine 0. 5mg /( kg·d),The analgesic drugs of dezocine group was dezocine 0. 5mg /( kg·d),The analgesic drugs of combined group was morphine 0. 25 mg /( kg·d) + dezocine 0. 25 mg /( kg·d). After surgery,compared the static and dynamic VAS score 6,12,24,48 h after surgery,analgesic effectiveness,remedy analgesic drug use and the adverse events of three groups. Results The static and dynamic VAS score 6,12,24,48 h after surgery,analgesic effectiveness,remedy analgesic drug use of three groups was no statistically significant( P > 0. 05). The adverse events of combined group was lower than that of morphine group and dezocine group,the difference was statistically significant( P < 0. 05). Conclusion Morphine combined with dezocine in postoperative analgesia has an good effect,and small doses of dezocine can enhance the analgesic effect of morphine,reducing the morphine consumption,reduce adverse reactions.更多还原
