【摘要】 目的:探索成年难治性癫患者的诊断、治疗及管理方法。方法:通过对就诊于我科的成年难治性癫痫患者的5年临床随访资料进行回顾性分析,了解难治性癫病情的转归;同时将几种联合用药方案的疗效进行对比,探索最佳方案。结果:确诊为难治性癫的52例患者中,29例(56%)继续到专科医院就诊;13例(25%)选择到社区或基层医院综合治疗;10例(19%)未规范治疗。经过正规抗癫痫治疗者有21例(40%),治疗效果明显,2年内未再发作;31例(60%)病情未能有效控制。结论:难治性癫痫患者的就诊、治疗明显影响患者病情的转归,治疗缺口大.依从性差,非医疗因素严重影响其身心健康。应尽快制定规范化的诊疗方式,加大宣传,着力改善癫患者的生活品质。目前几种常用的二线抗癫痫药物对于难治性癫痫的疗效比较差异无显著意义。更多还原

【Abstract】 Objective:To explore the diagnosis,treatment and management of adult patients with intractable epilepsy.Methods:The five-year clinical follow-up data of 52 cases with intractable epilepsy diagnosed in our department were analyzed retrospectively.The prognosis of the patients with intractable epilepsy was investigated.And the resalts of treatment with combined utilization of several antiepileptic drugs were compared to explore the best scheme.Results:Of 52 patients with intractable epilepsy,29 cases(55.8%) continued to be treated in the specialized hospital;13 cases(25.0%) chose community or local hospitals for a comprehensive treatment;10 cases(19.2%) had informal treatments.The treatment effect of 21 cases(40.4%) was obvious after regular anti-epilepsy treatment in 2 years without recurrence,in which 12 cases(44.4%) were controlled basically,and in 31 cases(59.6%),the disease could not be controlled effectively.Conclusion:The outcome of the patients with intractable epilepsy was significantly affected by the clinical condition and methods of treatment.A huge treatment gap and a poor compliance for those patients were obvious.Non-medical factors had a serious effect on their physical and mental health.Standardized diagnosis and treatment should be formulated as soon as possible,and standardised and professional treatments must be mades to improve the life quality of patients with epilepsy.There is no significant difference in the curative effect between several commonly used antiepileptic drugs of second line in treatment with intractable epilepsy.更多还原
