脂蛋白相关磷脂酶A2与原发性高血压患者发生缺血性卒中的相关性分析
Correlation analysis of occurrence of ischemic stroke between lipoprotein-associated phospholipase A2 and patients with essential hypertension
【摘要】 目的分析原发性高血压(EH)患者血清炎性因子脂蛋白相关磷脂酶A2(LP-PLA2)与缺血性卒中的关系,并探讨其临床意义。方法回顾性连续纳入符合纳入标准的EH患者167例,按照靶器官损伤(TOD)诊断标准分为单纯EH组79例,EH合并卒中组88例。测定每组入选对象的LP-PLA2水平,比较各组间临床相关资料和LP-PLA2水平的差异。采用多因素Logistic回归分析探讨LP-PLA2水平对EH患者发生缺血性卒中的临床意义。结果 Logistic回归分析结果显示,LP-PLA2是EH患者发生缺血性卒中的独立危险因素(OR=13.699,95%CI:5.23635.837,P<0.01);在入选的危险因素中,LP-PLA2的标化偏回归系数b’绝对值最大(b’=0.708)。结论 LP-PLA2作为新型的炎性标记物,是缺血性卒中重要的独立危险因素,对EH患者发生缺血性卒中的影响最大,可能为脑血管事件的预防和治疗提供新方向。
【Abstract】 Objectives To analyze the relationship between serum inflammatory factor lipoproteinassociated phospholipase A2( LP-PLA2) and ischemic stroke in patients with essential hypertension( EH)and to investigate its clinical significance. Methods A total of 167 consecutive patients with EH met the inclusion criteria were enrolled retrospectively. According to the target organ damage( TOD) diagnostic criteria,they were divided into either a simple EH group( n = 79) or a EH combined stroke group( n =88). The LP-PLA2 levels of the selected subjects in each group were detected. The differences of the clinical data and LP-PLA2 levels were compared between the 2 groups. Multivariate Logistic regression analysis was used to investigate the clinical significance of LP-PLA2 levels for EH patients occurring ischemic stroke.Results The results of Logistic regression analysis showed that Lp-PLA2 was an independent risk factor for ischemic stroke in patients with EH( OR,13. 699,95% CI 5. 236-35. 837,P < 0. 01). In the selected risk factors,the absolute value was maximum in the standardized partial regression coefficient b’ of Lp-PLA2( b’ = 0. 708). Conclusions As a new inflammatory marker,LP-PLA2 is an important independent risk factor for ischemic stroke. It has the greatest impact on the occurrence of ischemic stroke in patients with EH. It may provide a new direction for the prevention and treatment of cerebrovascular events.
