HMGB1与原因不明复发性流产的关系

【摘要】 目的探讨原因不明复发性流产(URSA)患者外周血中高迁移率族蛋白B1(HMGB1)的表达水平,及其对Th17/Treg平衡的影响。方法选取42例早孕期原因不明复发性流产患者为研究对象(URSA早孕组),并以32例正常早孕孕妇为正常早孕组(NP组)。采用流式细胞术(FCM)检测外周血中Th17和Treg细胞的比例;采用酶联免疫吸附实验(ELISA)检测外周血中HMGB1、RAGE、IL-17、IL-6、TGF-β及IL-10表达水平。结果 URSA早孕组外周血中Treg细胞比例低于正常早孕组(P<0.05),而Th17细胞比例和Th17/Treg比值均高于正常早孕组(P<0.05);URSA早孕组外周血HMGB1和RAGE表达高于正常早孕组(P<0.05);与正常早孕组相比,URSA早孕组外周血IL-17、IL-6表达水平均增高(P<0.05),而TGF-β、IL-10的表达水平均下降(P<0.05);在相关性分析比较中,URSA早孕组外周血HMGB1表达水平与RAGE、IL-17、IL-6均呈正相关,而HMGB1表达水平与TGF-β、IL-10均呈负相关。结论 URSA外周血中HMGB1表达水平增高,HMGB1可能通过直接或间接的方式调节Th17/Treg平衡模式参与URSA的发病机制。

【Abstract】 To investigate the expression and significance of the peripheral blood HMGB1 and its effect onTh17/Treg balance in unexplained recurrent spontaneous abortion(URSA), we recruited 42 cases of early pregnantwomen who had URSA history(URSA early pregnant group) and 32 normal early pregnant women with no history ofabortion(NP group). Flow cytometry was used to detect the frequency of Th17 and Treg cells in the peripheral bloodof both groups; ELISA was applied to detect the concentration of HMGB1, RAGE, IL-17, IL-6, TGF-β1 and IL-10 in the peripheral blood of both groups. Data showed that the frequency of Treg cells in CD4+T cells was lower inURSA early pregnant group than that of NP group(P<0.05), while the proportion of Th17 cells in CD4+T cells andthe ratio of Th17/Treg were higher in URSA group(P<0.05). Compared with NP group, the levels of HMGB1, RAGE,IL-17 and IL-6 in the peripheral blood of URSA early pregnant group were higher(P<0.05), while TGF-β1 andIL-10 levels were lower(P<0.05). The correlation analysis suggested that HMGB1 had positive correlation withRAGE, IL-17 and IL-6 in the peripheral blood, while HMGB1 was negatively correlated with TGF-β1 and IL-10(P<0.01) in URSA early pregnant group. Taken together, the happening of URSA may associated with Th17/Tregimbalance, and enhanced HMGB1 may participate in the pathogenesis of URSA through regulating the balance ofTh17/Treg in a direct or indirect way.
