【摘要】 目的为股直肌近端肌皮瓣向内旋转应用于临床腹股沟区软组织缺损的修复提供解剖学基础和提出新术式。方法动脉灌注红色乳胶的42侧成人下肢标本,解剖并观察旋股外侧动脉的降支进入股直肌的部位和在股直肌的分支、分布和吻合情况。结果股直肌近端由旋股外侧动脉提供血供,以降支为主。股直肌全长13.4±3.8cm,起止点直径为3.1±0.4mm、1.9±0.8mm;降支到股直肌有肌皮支2支以上,平均2.5支;股直肌的上支动脉(61.3±6.8)%和下支(91.0±5.6)%来自降支,且下支为股直肌的主要营养血管,下支起始处的外径及长度为2.4±0.4mm、7.6±2.3cm。结论股直肌近端带有旋股外侧动脉降支可以形成具有血管蒂长肌皮瓣;在股直肌近端离断髂前下棘的肌肉附着处并向下内旋转,临床可用于腹股沟部软组织缺损的填充。更多还原

【Abstract】 Objective To provide anatomical basis and put forward a new way of operation for proximal rectus femoris myocutaneous flap inward rotation applied to clinical inguinal region soft tissue defect repair. Methods 42 sides adult lower limb specimens with red latex artery perfusion were selected,and the observation of the location of descending branch of lateral femoral circumflex artery into the rectus femoris and the branch,distribution and anastomosis of descending branch of lateral femoral circumflex artery in the rectus femoris were made. Results TThe blood supply of proximal rectus femoris was offered by the lateral femoral circumflex artery,mainly supplied by descending branch. The rectus femoris is( 13. 4 3. 8) cm in length,the diameter of the start and stop points is 3.1 0. 4 mm and 1. 9 0. 8 mm,respectively. The descending branch usually has more than 2 muscular cutaneous branches to rectus femoris,average of 2. 5 branches. The upper branch artery( 61. 3 6. 8) % and the under branch artery( 91. 0 5. 6) % of the rectus femoris come from descending branch,the under branch artery of the rectus femoris is the main nutrient vessel,the origin of the under branch artery in the outer diameter and the length is 2. 4 0. 4 mm and 7. 6 2. 3 cm. Conclusions The proximal rectus femoris with the lateral femoral circumflex artery descending branch can be formed myocutaneous flap with vessel pedicle. the proximal rectus femoris can be used in the soft tissue defect of groin filling,in the anterior inferior spine place divide rectus femoris and inward rotation.更多还原
