新活素治疗难治性心力衰竭的临床疗效观察

【摘要】 目的 探讨分析比较新活素(rhBNP)与米力农治疗难治性心力衰竭的临床疗效。方法 收集2014年1月至2016年1月在心内科住院的难治性心衰患者120例,随机分为观察组和对照组。观察组予以rhBNP治疗,采用24h给药方式,以0.0075~0.01μg/(kg·min)静脉滴注,给药1次/周;对照组予以米力农治疗,以0.5μg/(kg·min)静脉滴注,给药1次/d。7d为1个疗程,观察30d。评估患者临床症状体征以及心功能分级,检测患者治疗前后收缩压、舒张压、心率、尿量、分析左室射血分数(LVEF)、血清氨基末端脑钠肽前体(NT-proBNP)浓度。结果rhBNP、米力农可显著改善患者心衰症状。与对照组比较,难治性心衰患者经rhBNP治疗后临床症状以及心功能分级明显改善,LVEF显著升高(P<0.05),血清NT-proBNP浓度显著降低(P<0.05)。结论 rhBNP、米力农能明显改善难治性心衰患者心功能,且rhBNP疗效优于米力农。

【Abstract】 Objective To investigate the clinical efficacy of rhBNP and milrinone in the treatment of refractory heart failure.Methods 120 patients with refractory heart failure from First Affiliated Hospital of Soochow University were enrolled for this study between January 2014 and January 2016.All the patients were randomly divided into observation group and control group.The observation group was treated with rhBNP given by intravenous infusion of 0.0075~0.01 μ g /(kg-min) once daily.The control group underwent routine anti heart failure treatment of milrinone which was given by intravenous infusion of 0.5 μ g /(kg-min) once a day.We observed for 30 days as 7 days a period of treatment.Symptoms,NYHA,Systolic pressures,diastolic pressures,heart rate,urine volume,LVEF,plasma levels of NT-proBNP were detected in two groups before and after treatment.Results After one course of treatment,the two groups showed significant improvement of clinical symptoms.Compared with control group,the rhBNP treated patients showed significant statistical difference in clinical symptom improvement,NYHA and showed higher LVEF(P<0.05),lower plasma levels of NT-proBNP(P<0.05).Conclusion The treatment of rhBNP and milrinone significantly improved the clinical symptoms of the patients with refractory heart failure,and rhBNP was proved to be more effective.
