附加导联在诊断右束支传导阻滞中意义的探讨

【摘要】 目的 探讨心电图附加导联在诊断右束支传导阻滞(RBBB)中的意义。方法 观察450例正常人心电图V1′′、V1′′、V1′′′及V3R导联QRS波形态及演变规律。结果 V1′→V1”→V1′′′QRS波演变趋势:ri波振幅逐渐降低,出现r’波的概率及r’波振幅逐渐增高,即出现r′>r波概率越来越高。V3R导联元1例出现r′波。V1′、V1′′、V1′′′及V3R导联出现RBBB图形分别是18例(4%)、105例(23%)、193例(43%)、0例(0%)。结论 附加导联作为RBBB早期诊断,V1′、V1′′、V1′′′可能存在假阳性;而V3R导联则未见假阳性,具有较高的可靠性。更多还原

【Abstract】 Objective To investigate the significance of additional leads in diagnosis of right bundle branch block in ECG.Methods 450 cases of normal V1’,V1’’,V1’ " and V3 R lead QRS waveform morphology and evolution were observed and evaluated.Results V1’→V1" →V1’ "QRS waveform trend is:r wave amplitude decreased,therefore,the probability r’ waves emergence and amplitude of r’ wave both increased,ie.the probability of r’> r wave probability was gradually increasing.In lead V3 R,there was no r ’ wave.Cases of V1’,V1",V1’ " and V3 R lead that had RBBB graphics were 18 cases(4%),105 cases(23%),193 cases(58%),0 cases(0%),respectively.Conclusion When using additional leads in early diagnosis of RBBB,V1’、 Vl" 、 V1’’’may have false positive but V3 R did not.Through this study,we can conclude that V3 R shows a higher reliability in RBBB early diagnosis.更多还原
