【摘要】 目的提高对脑淀粉样血管病相关性炎症(CAA-Ⅰ)的认识。方法回顾性描述1例CAA-Ⅰ患者的临床表现和影像学特点,并复习相关文献进行分析。结果患者为中老年男性,以头痛为临床表现,MRI提示多发非对称性白质高信号病灶并延伸至皮质下白质,SWI提示颅内散在皮质、皮质下多发微出血病灶,基因检测为Apo Eε4/ε4,11C标记的匹兹堡复合物B PET检测阳性,经验性大剂量激素冲击治疗后颅内病灶明显改善。结论 CAA-Ⅰ病例报道较少,临床医生对此病的认识不足。多序列的MRI检查(尤其是SWI)可提高CAA-Ⅰ的诊断率。

【Abstract】 Aim To improve the recognition of cerebral amyloid angiopathy-related inflammation(CAA-I). Methods The clinical manifestation and imaging characteristics of one case of cerebral amyloid angiopathy-related infl ammation was retrospectively described. Related literature were reviewed and analyzed. Results The patient is a mid-aged male with chief complaint of headache. MRI showed bilateral asymmetric white matter hyperintensities extending to subcortex. SWI demonstrated sporadic cortical and subcortical microbleeds. Apo E gene is ε4/ε4. The 11C-Pittsburgh compound B PET examination was positive. The white matter hyperintensities were significantly relieved after empirical pulse steroid therapy. Conclusion Case reports of CAA-I were rare, so clinical practitioners had insuffi cient recognitionof this disease. The multimodal MRI, especially SWI, can improve the diagnostic rate of this disease.
