2型糖尿病患者不稳定糖化血红蛋白与7时点血糖波动的相关性

【摘要】 目的通过对2型糖尿病(T2DM)患者不稳定糖化血红蛋白(LA1C)与7个时间点平均血糖波动幅度的相关研究,分析LA1C作为血糖波动评价指标的价值。方法分析373例住院T2DM患者的LA1C与7个时间点血糖的平均血糖波动幅度(MAGE)、平均血糖标准差(MGS)的相关性;比较LA1C不同水平患者组间MAGE、比较不同空腹血糖(FBG)和不同糖化血红蛋白(HbA1c)患者组间LA1C和MAGE的变化。结果 T2DM患者LA1C与MAGE、MGS间呈正相关(r分别为0.308、0.322,P<0.05)。LA1C≥1.68%组的MAGE高于LA1C<1.68%组(Z=3.386,P<0.05),FBG<6.5mmol/L组的LA1C和MAGE低于FBG(6.5~11.0)mmol/L组和FBG>11.0 mmol/L组(Z=11.16~11.88,P均<0.05;Z=2.52~4.98,P均<0.05),HbA1c<6.5%组LA1C和MAGE低于HbA1c 6.5%~9.0%组、HbA1c 9.0%~11.0%组和HbA1c>11.0%组(Z=2.87~4.55,P均<0.05;Z=3.11~5.40,P均<0.05)。结论 T2DM患者LA1C水平与患者7个时间点平均血糖波动幅度相关,可以作为评价血糖波动的简易指标。

【Abstract】 Objective To study the correlation of labile hemoglobin a1c( LA1C) with the mean amplitude of glucose excursion of 7points blood glucose,and analyze the tested results of LA1C for evaluating the glucose variations in type 2 diabetes mellitus. Methods A total of 373 cases of hospitalized patients with type 2 diabetes mellitus were enrolled. The correlation between LA1C and the mean amplitude of glucose excursion( MAGE),mean standard deviation( MSD) were analyzed and the differences of MAGE between the group LA1C≥1. 68% and the group LA1C < 1. 68% was compared. The differences of LA1C and MAGE between normal FBG and abnormal FBG groups,between normal HbA1c and abnormal HbA1c groups were compared. Results Positive correlations between the level of LA1C and MAGE( r = 0. 308,P < 0. 05),LA1C MGS( r = 0. 322,P < 0. 05) were found. The MAGE level in the group of LA1C≥1. 68 was significantly higher than that of LA1C < 1. 68( Z = 2. 024,P < 0. 05). The LA1C and MAGE levels in the group of FBG < 6. 5 mmol / L were significantly lower than those of both FBG( 6. 5 to 11. 0) mmol / L and FBG > 11. 0 mmol / L( Z = 11. 16-11. 88,P < 0. 05; Z = 2. 52-4. 98,P < 0. 05). The LA1C and MAGE in the group of HbA1c < 6. 5% were significantly lower than that in Hb A1c( 6. 5% to 9. 0%),HbA1c 9. 0% to 11. 0% and HbA1c > 11. 0%( Z = 2. 87-4. 55,P < 0. 05; Z = 3. 11-5. 40,P < 0. 05).Conclusion The level of LA1C was closely correlated to the MAGE of 7 point blood glucose,so LA1C may be used as a simple parameter for evaluating the glucose variations in type 2 diabetes mellitus.
