Toll样受体4与乙型肝炎的相关性

【摘要】 Toll样受体(TLR)4是一种重要的模式识别受体,在抑制HBV感染中发挥着重要的作用。TLR4识别HBV相关蛋白分子,经髓系分化因子88(My D88)和非My D88途径,产生肿瘤坏死因子α、白细胞介素(IL)6、IL-12等炎症因子和Ⅰ型干扰素而发挥抗病毒作用。同时发现TLR4可通过调节辅助性T淋巴细胞(Th)1与Th2间的平衡,促进CD8+T淋巴细胞在肝脏粘附及诱导一氧化氮合酶发挥抑制HBV作用。若TLR4过表达,会通过机体免疫系统过度激活,加重肝脏的缺血再灌注及肝星状细胞激活而加重肝脏炎症。回顾了国内外近年相关文献,探讨了TLR4在乙型肝炎发病中的相关机制,总结了TLR4与乙型肝炎的相关性,以期通过TLR4与乙型肝炎相关性的研究,发现治疗乙型肝炎及延缓其向肝硬化和肝癌进展的新方案。

【Abstract】 Toll- like receptor 4( TLR4) is an important pattern recognition receptor and plays an important role in the inhibition of hepatitis B virus( HBV) infection. It identifies the HBV- related proteins and produces inflammatory factors,such as tumor necrosis factor- α,interleukin- 6,and interleukin- 12,and type I interferon through the myeloid differentiation factor 88( My D88) and the non- My D88 pathway,and thus TLR4 exerts its antiviral effect. Meanwhile,it has been found that TLR4 regulates the balance between T helper 1 and T helper 2 cells,promotes the adhesion of CD8+T lymphocytes in the liver,induces the secretion of inducible nitric oxide synthase,and thus inhibits HBV. If overexpressed,TLR4 will aggravate liver inflammation by excessively activating the body’s immune system,aggravating liver ischemia / reperfusion,and activating hepatic stellate cells. This article reviews related articles published at home and abroad in recent years,discusses the mechanisms of action of TLR4 in the pathogenesis of hepatitis B,and summarizes the association between TLR4 and hepatitis B,in order to find new therapies for hepatitis B and new protocols which delay the progression of hepatitis B to liver cirrhosis and liver cancer through the studies on the association between TLR4 and hepatitis B.
