【摘要】 目的通过研究使用对比剂钆喷酸葡胺（Gd-DTPA）增强前/后肝实质及肝内常见恶性病变T₂WI信号的变化,探讨优化肝脏磁共振成像增强扫描流程的可能性。方法收集正常对照组89例和临床诊断明确的肝内常见恶性病变42例（其中肝癌19例、肝转移瘤23例）,增强前后分别行T₂WI序列扫描,对T₂WI信号变化进行测量对比分析。结果正常肝实质、原发性肝癌、肝内转移瘤增强后T₂WI信号轻度减低,其差异存在统计学意义（P<0.01）。对正常肝实质、肝癌、肝转移瘤3组间增强扫描前后T₂WI信号差值进行对比,组间差异存在统计学意义（P<0.05）;但信号下降百分比差异无统计学意义（P>0.05）。增强前后对比,肝内常见恶性病变T₂WI信号值与其周围正常肝实质T₂WI信号差值差异无统计学意义（P>0.05）。结论正常肝实质及肝内常见恶性病变因受对比剂影响,增强后T₂WI强化程度减低,但对于肝内常见恶性病变的检出影响不大,从而说明增强后行MRI检查是可行的。更多还原

【Abstract】 Objective To explore the possibility of optimizing liver MRI enhancement scanning process,the changes of T₂WI signal of hepatic parenchyma and intrahepatic common malignancy were studied before and after using contrast agent gadolinium diethylenetriamine pentaacid（Gd-DTPA.）Methods Eighty-nine cases were collected normally in the control group and 42 cases which were clinically diagnosed as intrahepatic common malignancy（19cases of liver cancer and 23 cases of liver metastases）,the changes of T₂WI signal were measured and analyzed comparatively.Results Contrast agent Gd-DTPA was used to scan normal liver,liver cancer and liver metastases,T₂WI signal reduced slightly.The difference was statistically significant（P <0.01）.The difference of T₂WI signal before enhancement and after enhancement were compared among groups of normal liver and liver cancer and liver metastases,there were statistically significant difference（P <0.05）.However,there was no significant difference on the percentage of signal reduction（P >0.05）.By comparing the signal difference before and after enhancement,T₂WI signal difference between common malignant lesions in liver and its surrounding normal liver parenchyma was not significantly different（P >0.05）.Conclusion Hepatic parenchyma and intrahepatic common malignancy were affected by contrast agent,the enhanced degree of T₂WI signal was decreased after the enhancement,but it had little effect on the detection of common malignant lesions in liver.Therefore,it is feasible to perform T₂WI after enhancement.更多还原
