高频超声对毛母质瘤和表皮样囊肿的鉴别诊断价值

【摘要】 目的探讨超声对毛母质瘤和表皮样囊肿的鉴别诊断价值。方法回顾性分析我院行超声检查并经病理证实的31例毛母质瘤患者(毛母质瘤组)和35例表皮样囊肿患者(表皮样囊肿组)的临床和超声资料。对比分析两组的病灶部位、大小、形状、纵横比、周边特征、内部回声、后方回声特征及血流分布情况。结果两组患者在病灶部位、大小、形状及纵横比方面比较,差异均无统计学意义;在年龄、性别、周边特征、内部钙化灶、后方回声特征及血流分布方面比较,差异均有统计学意义(均P<0.05)。毛母质瘤超声表现为内部伴或不伴钙化灶的皮下浅层低回声病灶,周边可见低回声晕征,部分病灶可见血流信号;表皮样囊肿超声表现为皮下浅层较均匀低回声病灶,边界清晰,后方伴回声衰减。结论高频超声有助于毛母质瘤和表皮样囊肿的鉴别诊断,具有重要的临床价值。

【Abstract】 Objective To explore the value of ultrasound in differential diagnosis of pilomatricoma(PM)and epidermoid cyst(EC).Methods Retrospectively analyzed the characters of clinic and ultrasound of 31 patients with PM(PM group)and 35 patients with EC(EC group),they were confirmed by pathology. The lesion size,shape,aspect ratio,the peripheral features,internal echo,posterior echo characteristics and distribution of blood flow were analyzed comparatively between two groups.Results There was no significant difference between PM and EC group in aspects of lesions’ location,size,shape and aspect ratio,and there was significant difference in the patients’ age,gender,peripheral feature,calcification,posterior echo and blood flow(all P<0.05).For ultrasonic characterization,PM presented with hypoechoic lesions with or without internal calcification in superficial tissue,peripheral hypoechoic halo sign was observed,blood flow signal were found in some lesions.Epidernoid cyst demonstrated homogeneous hypoechoic lesions in superficial layer with clear border and posterior acoustic enhancement.Conclusion High frequency ultrasound is helpful in the differential diagnosis of PM and EC.
