二维斑点追踪成像技术对冠状动脉粥样硬化性心脏病患者左室整体收缩功能和同步性的研究

【摘要】 目的应用二维斑点追踪成像(2D-STI)技术检测心肌缺血和心肌梗死患者左室整体收缩功能和同步性的变化,以探讨其临床应用价值。方法 46例心肌缺血患者(缺血组)、44例心肌梗死患者(梗死组)和60例健康志愿者(对照组)均行2D-STI和常规超声心动图检查,获取左室整体纵向应变、环向应变、径向应变及16节段纵向应变达收缩峰值时间的标准差(Tls-l6-SD)、12节段径向、环向应变达收缩峰值时间的标准差(Trs-l2-SD、Tcs-l2-SD),各组间进行对比分析。结果与对照组比较,缺血组左室整体纵向应变明显降低(P<0.05),Tls-16-SD明显延长(P<0.05)。梗死组左室收缩功能参数及同步性参数与缺血组和对照组比较差异均有统计学意义(均P<0.05),收缩同步性参数均与左室射血分数呈负相关。结论 2D-STI技术可早期客观评价心肌缺血患者左室收缩功能和同步性,能准确测量心肌梗死患者左室收缩功能和同步性,左室不同步运动可影响其收缩功能。

【Abstract】 Objective To explore the clinical value of two-dimensional speckle tracking imaging(2D-STI) in evaluating left ventricular global systolic function and synchrony in patients with myocardial ischemia and myocardial infarction.Methods Forty-six patients with myocardial ischemia(myocardial ischemia group),44 patients with myocardial infarction(myocardial infarction group) and 60 healthy subjects(control group) were underwent 2D-STI and conventional echocardiography. Left ventricular overall longitudinal strain(GLS),loop strain,radial strain,16 segment longitudinal strain systolic peak time standard deviation(Tls-l6-SD),12 segment radial,loop strain systolic peak time standard deviation(Trs-l2-SD,Tcs-l2-SD) were acquired and analyzed between each group. Results Compared with control group,GLS was significantly decreased,Tls-16-SD was significantly prolonged in myocardial ischemia group(P <0.05).Compared with the control group and myocardial ischemia group,all the parameters of systolic function and synchrony had statistical differences in myocardial infarction group(all P < 0. 05). LVEF were negative correlated with the systolic synchrony parameters. Conclusion 2D-STI can early assess left vertricular systolic function and synchrony in patients with myocardial ischemia,and accurately measure left vertricular systolic function and systolic synchrony in patients with myocardial infarction. Left vertricular systolic asynchrony has negatively effect on left ventricular systolic function.
