小视野多b值DWI双指数模型在甲状腺良恶性结节鉴别中的应用
The Application of The Small Field-of-View Biexponential Model of Multi B-Value Diffusion Weighted Imaging in Differentiating Benign of Thyroid Nodules from Malignant
【摘要】 目的探讨小视野多b值扩散加权成像双指数模型各参数对甲状腺良恶性结节的鉴别诊断价值。方法对52例甲状腺结节(共85个)患者行多b值DWI序列扫描,依据病理结果分为良性组及恶性组,比较其双指数模型各参数(Slow ADC、Fast ADC、f值)有无统计学差异。结果良性结节Slow ADC、Fast ADC、f值分别为(1.588±0.407)×10-3mm2/s,(153.548±99.111)×10-3mm2/s,(20.984±8.568)%;恶性结节Slow ADC、Fast ADC、f值分别为(0.959±0.228)×10-3mm2/s,(171.392±104.186)×10-3mm2/s,(22.620±8.491)%。其中Slow ADC有统计学差异,Fast ADC、f值差异无统计学意义。Slow ADC值ROC曲线下面积为0.930,敏感度、特异度分别为90.00%,95.38%。结论小视野多b值DWI双指数模型对甲状腺良恶性结节有鉴别诊断价值。
【Abstract】 Objective To assess the diagnostic value of small field-of-view biexponential model of multi b-value diffusion weighted imaging in differentiating benign and malignant thyroid nodules. Methods 85 thyroid nodules from 52 patients underwent multi b-value DWI. Thyroid nodules were divided into malignant and benign groups based on the pathological findings. Compare Slow ADC,Fast ADC,f values of both groups to see whether there are statistical differences. Results The Slow ADC,Fast ADC,f values in benign group were( 1. 588±0. 407) ×10-3mm2/ s,( 153. 548 ±99. 111) ×10-3mm2/ s,( 20. 984 ±8. 568) %. The Slow ADC,Fast ADC,f values in malignant group were( 0. 959±0. 228) ×10-3mm2/ s,( 171. 392±104. 186) ×10-3mm2/ s,( 22. 620 ±8. 491) %.There were statistically significant differences of the Standard ADC and Slow ADC between two groups. There was no statistically significant difference of the Fast ADC and f values in both groups. The area under the ROC curve for Slow ADC was 0. 930. Sensitivity and specificity were 90. 00% and 95. 38%. Conclusion Small field-of-view biexponential model of multi b-value diffusion weighted imaging has diagnostic value for differentiating benign and malignant of thyroid nodules.
