原发性干燥综合征患者外周血B细胞瘦素表达水平及其临床意义

【摘要】 目的 :探讨原发性干燥综合征(pSS)患者外周血B细胞瘦素(LP)及瘦素受体(LPR)的表达及临床意义。方法:应用流式细胞仪(FCM)检测56例pSS患者及19例正常对照者外周血B细胞LP及LPR的表达,分析LP及LPR表达与患者临床特点及疾病活动的相关性。结果:pSS患者B细胞LP表达与正常对照组差异无统计学意义(P>0.05),而LPR表达高于正常对照组,差异有统计学意义(P=0.029)。活动期患者B细胞LP表达与稳定期差异无统计学意义(P>0.05),而LPR表达高于稳定期,差异有统计学意义(P=0.037)。患者B细胞LP表达与LPR表达呈正相关,具有统计学意义(r=0.332,P=0.004)。患者B细胞LP表达与低密度脂蛋白(r=-0.276,P=0.030)、肌酐(r=-0.256,P=0.034)呈负相关,具有统计学意义。患者B细胞LPR表达与体重(r=-0.294,P=0.010)、BMI(r=-0.296,P=0.010)呈负相关,具有统计学意义。口腔溃疡组B细胞LPR表达高于无口腔溃疡组,差异有统计学意义(P<0.01)。结论:pSS患者B细胞LPR存在异常,与病情活动相关,B细胞上LP与LPR显著正相关,可以调节脂肪代谢及体重,可能参与到pSS疾病中肾脏损伤及口腔溃疡。

【Abstract】 Objective:To investigate the expression and clinical significance of leptin(LP) and leptin receptor(LPR)in B cell in patients with primary Sj觟gren’s syndrome(pSS). Methods:The method of Flow Cytometry was used to measure the expression of LP and LPR in B cell in 56 patients with pSS and the test results were compared with those of 19 normal controls. The linear correlation analysis was adopted to evaluate the relationship between the expression of LP and LPRin B cell and clinical manifestations. Results:There was no significant difference in the expression of LP in CD19+ B cell between pSS patients(6.53±6.67) and the normal controls(4.63±2.90)% as well as active pSS patients(7.17±9.19)% and inactive patients(5.98 ±3.28)(P >0.05). The expression of LPR in CD19 + B cell was significantly higher in pSS patients(25.21±18.33)% than in the normal controls(17.93±9.32)%,(P=0.029), and significantly higher in active pSS patients(30.65±20.45)% than inactive patients(20.49±15.08)%(P=0.037). The expression of LP was positively correlated with the expression of LPR in CD19+ B cell(r=0.332,P=0.004). The expression of LP in CD19+ B cell was negatively correlated with the serum level of Low density lipoprotein(r=-0.276,P=0.030)and creatinine(r=-0.256,P=0.034) significantly. The expression of LPR in CD19+ B cell was negatively correlated with weight(r=-0.294,P=0.010) andbody mass index(BMI)(r=-0.296,P=0.010)significantly. The expression of LPR in CD19+ B cell in the group with oral ulcer was significantly higher than those without oral ulcer(P<0.01). Conclusion:There is LPR in CD19+ B cell imbalance in pSS patients, and this is correlated with pSS activity indexes. The expression of LPis positively correlated with the expression of LPR in CD19 + B cell.LP and LPR can regulate fat metabolism and body weight, which may be involved in renal injury and oral ulcers in pSS.It may provide new therapeutic targets for treatment of pSS.
