SWI对脑梗死侧支循环的诊断及临床意义

【摘要】 目的 :探讨3.0 T MRI的SWI对脑梗死侧支循环的诊断及临床意义。方法 :选择35例经临床和影像确诊为脑梗死的患者均行常规头颅MRI、DWI、MRA、SWI检查,10例行SWI复查。观察脑梗死的常规MRI和SWI表现,以及脑梗死区侧支循环的显示程度,得出相应临床NIHSS评分,并进行统计分析。结果:35例脑梗死SWI显示侧支循环的程度:0级9例(25.7%),1级10例(28.6%),2级16例(45.7%)。侧支循环血管显示程度与临床NIHSS评分有明显相关性(r=0.668,P<0.01)。结论:SWI能够反映脑血管病变中的血氧饱和度水平改变情况,以及脑梗死是否有侧支循环形成和责任血管情况,对临床制订治疗方案和预后判断具有重要意义。

【Abstract】 Objective:To investigate the effects of 3.0 T magnetic resonance susceptibility weighted imaging(SWI) on cerebral infarction diagnosis and clinical significance of collateral circulation. Methods:35 cases of clinical and imaging diagnosis of cerebral infarction,the male 22 cases,13 cases of female. Patients were performed conventional head MRI,MRA,SWI,including10 patients with SWI review. To observe the performance of cerebral infarction on conventional MRI and SWI,to observe the demonstration of collateral circulation at infarction area,and draw the corresponding clinical NIHSS score. Statistical analysis was performed. Results:In 35 cases of cerebral infarction,SWI classification of the degree of collateral circulation,grade 0,9 cases(25.7%),grade 1,10 cases(28.6%),grade 2,16 cases(45.7%). Degree of collateral circulation blood vessels showed obvious correlation with clinical NIHSS score(r=0.668,P< 0.01). Conclusion:SWI imaging can reflect changes in the blood oxygen saturation level in the cerebrovascular disease,also could show collateral circulation formation,offending vessels of cerebral infarction.It could make correct decisions for clinical treatment and prognosis judgment,and has important clinical significance.
