自制呼吸套件用于新生儿麻醉呼吸管理的临床研究

【摘要】 目的研究自制呼吸套件用于新生儿胸腹部手术气管内全麻中呼吸管理的可行性。方法方便选择需气管内全麻的新生儿胸腹部手术50例,于气管插管前30 min(T0)、插管并连接呼吸套件行辅助通气后半小时(T1)分别采集桡动脉血进行血气分析。分别记录应用自制呼吸套件辅助通气前后患儿SPO2、RR、Pa O2、Pa CO2、HR、MAP及体温。结果Pa O2在T1时间点(421.8±21.3)mm Hg高于T0(83.9±3.7)mm Hg(P<0.01),PETCO2在T1时间点(34.4±4.2)mm Hg低于T0(41.2±4.0)mm Hg(P<0.01);其余指标与插管前相比均差异无统计学意义(P>0.05)。结论自制呼吸套件用于新生儿胸腹部手术气管内全麻中的呼吸管理,安全有效,操作简便易行,可控性好,适于基层医疗单位推广使用。

【Abstract】 Objective To study the feasibility of respiratory management with self-made breathing kit for thoracic and abdominal operation on newborn under intratracheal general anesthesia. Methods 50 patients undergoing thoracic and abdominal operation on newborn were administrated intratracheal general anesthesia, and collecting radial artery blood to carry out blood gas analysis at half an hour before trachea cannula(T0) and half an hour after trachea cannula(T1), and connecting breathing kit to assist ventilation. Recording SPO2, RR, Pa O2, Pa CO2, HR, MAP as well as temperature before and after applying self-made breathing kit to assist ventilation. Results The Pa O2(421.8±21.3)mm Hg at T1 was significant higher than(83.9±3.7)mm Hg at T0(P<0.01); The PETCO2(34.4±4.2)mm Hg at T1 was lower than(41.2±4.0)mm Hg at T0(P<0.01); while other indicators were no statistical significance than pre-trachea cannula(P>0.05). Conclusion Self-made breathing kit,using respiratory management of thoracic and abdominal operation on newborn undergoing intratracheal general anesthesia,is safe, effective and easy to administrate. It has controllability and is suitable to be popularized and used in primary medical units.
