东阳地区肺癌患者肺部感染的致病菌及耐药性分析
An analysis of the pathogenic bacteria causing lung infection and their drug resistance in patients with lung cancer in Dongyang region
【摘要】 目的了解东阳地区肺癌患者肺部感染的致病菌和耐药情况,为抗菌药物使用提供依据。方法收集近2年所有肺癌住院患者下呼吸道感染的痰标本进行病原菌培养,分析病原菌种和耐药性。结果共分离出206株致病菌,以真菌为多共116株(占56.31%),依次为白色假丝酵母菌(42.23%)、光滑假丝酵母菌(11.17%);革兰阴性菌71株(占34.47%),以肺炎克雷伯菌(19.40%)、大肠埃希菌(5.34%)和铜绿假单胞菌(3.34%)居多,对头孢曲松、头孢唑啉、氨苄西林的耐药率较高,对亚胺培南、头孢哌酮/舒巴坦的耐药率较低;革兰阳性菌19株(9.22%),对青霉素、红霉素、氨苄西林的耐药率较高,对利奈唑烷、万古霉素的耐药率较低。结论肺癌患者所感染致病菌对各种常规抗生素均表现出较强的耐药性;使用抗生素前尽早做病原菌培养,根据药敏结果合理使用抗菌药物,减少耐药株产生。
【Abstract】 Objective To understand the pathogenic bacteria that cause lung infection in lung cancer patients and their drug resistance so as to obtain knowledge for scientific use of antimicrobial agents. Method Firstly,the sputum specimen from all lung cancer patients with lower respiratory tract infection hospitalized over the past two years were collected and cultivated for pathogenic bacteria. Then,the species of pathogenic bacteria species and their drug resistance were analyzed.Findings A total of 206 strains pathogenic bacteria was obtained,most( 56. 31%) of which were fungi( 116 strains),followed by white candida( 42. 23%),and smooth candida( 11. 17%). 34. 47% were gram-negative bacteria( 71strains) where 19. 40% were klebsiella pneumoniae and 5. 34% were e. coli and 3. 34% were pseudomonas aeruginosa.They had high resistance to ceftriaxone, cefazolin, and ampicillin and low resistance to imipenem, cefoperazone /sulbactam. 19 strains( 9. 22%) were gram positive bacteria which had high resistance to penicillin,erythromycin,and ampicillin and low resistance to linezolid,vancomycin resistant rate is low. Conclusion The pathogenic bacteria that cause lung infection in patients with lung cancer patients were found to have strong resistance to conventional antibiotics. Before any antibiotics are used,cultivation of pathogenic bacteria should be conducted and use of antibiotics should be in accordance with susceptibility so as to deter the development of bacteria with high resistance to drugs.
