骨质疏松所致腰椎骨质并脊髓神经损伤的规范治疗

【摘要】 目的观察后路减压AF系统内固定术治疗骨质疏松所致腰椎骨质破坏并脊髓神经损伤的治疗效果。方法选取我院收治的60例骨质疏松性腰椎骨折并脊髓神经损伤患者为研究对象,随机分为对照组和观察组各30例,对照组行后路全椎板切除减压术,观察组行后路减压AF系统内固定术。结果术前2组伤椎前缘及后缘高度比较均无显著性差异(P>0.05),术后观察组伤椎前缘及后缘高度均较术前明显改善(P<0.05),而对照组术前术后无显著性差异(P>0.05)。实验组手术优良率86.7%,对照组为53.3%,实验组手术优良率高于对照组,差异有统计学意义(P<0.05)。2组术后神经功能恢复情况比较,观察组显著优于对照组(P<0.05)。结论后路减压AF系统内固定术治疗骨质疏松性腰椎骨折并脊髓神经损伤在恢复椎体高度、手术疗效及神经功能恢复方面效果满意,可于临床推广应用。

【Abstract】 Objective To evaluate the clinical effects of the application of posterior decompression and AF system internal fixation operation on lumbar bone destruction and spinal nerve injury caused by osteoporosis.Methods Sixty cases with lumbar bone destruction and spinal nerve injury caused by osteoporosis in our hospital were selected and randomly divided into observation group and control group,30 cases in each group.Cases in the control group were given posterior laminectomy and others in the observation group were treated with posterior decompression and AF system internal fixation operation.Results Before operation,the heights of posterior and anterior edge of the injured vertebra between two groups showed no significant differences(P>0.05),which was obviously improved in the observation group(P<0.05)after operation but not in the control group.Additionally,26cases(86.7%)in the observation group and 16cases(53.3%)in the control group acquired excellent operation effect,with statistical difference(P<0.05).After treatment,the observation group showed better postoperative neurologic recovery than the control group(P<0.05).Conclusion Posterior decompression and AF system internal fixation operation can exert an effective influence on the recovery of vertebral height,operative efficacy and nerve function recovery in patients with lumbar bone destruction and spinal nerve injury caused by osteoporosis,which should be tried out in clinical application.
