【摘要】 目的:对急性白血病患者不同时期行PICC置管的效果进行观察,并观察并发症发生情况。方法:将某医院2013年1月-2015年5月接收的急性白血病患者82例作为观察对象,按PICC置管时期不同分为对照组与观察组,对照组40例患者在入院后第一次化疗方案出来后立即给予PICC置管,观察组42例患者在接受化疗2疗程后,外周血与骨髓细胞形态以及临床症状等均达到正常后再行PICC置管,观察2组患者PICC置管后并发症发生情况,以及血小板计数、白细胞计数、红细胞计数变化。结果:观察组与对照组PICC置管后发生静脉炎、感染、出血、静脉血栓形成、导管堵塞、非计划性拔管等并发症的机率分别为9.52%,22.50%,2组比较,差异有统计学意义(P<0.05);观察组患者血小板计数、白细胞计数及红细胞计数与对照组比较,差异有统计学意义(P<0.05)。结论:不同时期行PICC置管,并发症发生机率不同,接受2疗程化疗后给予血小板、白细胞以及红细胞计数进行纠正后行PICC置管的急性白血病患者,并发症发生机率较小。更多还原

【Abstract】 Objective: To analyze effects of PICC catheter during different periods on complications of patients with acute leukemia.Methods: 82 patients with acute leukemia admitted from January 2013 to May 2015 were selected as research subjects,who were divided into a control group and an observation group according to different PICC cathetering periods. 40 patients in the control group underwent PICC cathetering immediately following chemotherapy scheme after admission,and 42 patients in the observation group received PICC cathetering when peripheral blood,bone marrow cell morphology and clinical symptoms reached normal after 2 courses of chemotherapy. Complications occurrence and changes in platelet count,white blood cell count and red blood cell count of the patients after PICC cathetering were observed. Results: For complications such as phlebitis,infection,hemorrhage,venous thrombosis,catheter blockage and unplanned extubation,the incidence was 9. 52% and 22. 50% respectively for the observation group and the control group after PICC cathetering. The difference was statistically significant( P < 0. 05). Comparing platelet count,white blood cell count and red blood cell count between the two groups,the difference was statistically significant( P < 0. 05). Conclusion: PICC cathetering in different periods may result in different chances of complications. After two cycles of chemotherapy and correction of platelets,white blood cell and red blood cell count,PICC cathetering in patients with acute leukemia may contribute to smaller chance of complications.更多还原
