【摘要】 目的研究全胸腔镜与后外侧开胸手术对非小细胞肺癌患者的早期创伤指标及免疫功能的影响。方法选取手术治疗的非小细胞癌肺癌患者56例,按手术方式的不同分组,接受后外侧开胸手术治疗的28例患者为对照组,接受全胸腔镜手术治疗的28例患者为观察组,对比2组的临床效果,指标包括术中出血量、术中输血量、手术时间及住院时间等,并对比2组治疗前后炎性因子及免疫细胞水平的变化。结果观察组术中出血量、术中输血量、手术时间及住院时间均优于对照组,差异有统计学意义（P<0.05）;观察组术后IL-6、IL-2R、CRP、SAA等炎性因子水平上升程度较对照组低,差异有统计学意义（P<0.05）;观察组术后T淋巴细胞中CD₄⁺、CD₈⁺、CD₄⁺/CD₈⁺等指标优于对照组,差异有统计学意义（P<0.05）。结论对非小细胞肺癌患者的治疗,早期采取全胸腔镜手术治疗的效果较好,且改善患者机体免疫功能,值得在临床中推广使用。更多还原

【Abstract】 Objective To investigate the effects of whole thorascopic surgery and open chest surgery on early indexes of trauma and immune function in patients with non small cell lung cancer（ NSCLC）. Methods Fifty-six patients with NSCLC who underwent surgical treatment in our hospital from June 2012 to August 2015 were divided into two groups according to different surgical modes. The 28 patients who received posterolateral thoracotomy were served as observation group,and the other 28 patients who received whole thorascopic surgery were served as control group. The therapeutic effects including intraoperative bleeding volume,intraoperative blood transfusion volume,operation time and hospitalization time were observed,moreover,the changes of inflammatory factors levels and immune cells levels before and after treatment were detected and compared between two groups. Results The intraoperative bleeding volume,intraoperative blood transfusion volume,operation time and hospitalization time in observation group were superior to those in control group（ P < 0. 05）. After treatment the levels IL-6,IL-2R,CRP,SAA in observation group were significantly lower than those in control group（ P <0. 05）. The immune function indexes including CD₄⁺,CD₈⁺,CD₄⁺/CD₈⁺ in observation group were superior to those in control group（ P < 0. 05）. Conclusion The whole thorascopic surgery has better therapeutic effects on NSCLC at early period,which can improve patient’s immune function,thus it is worth spreading application in clinic.更多还原
