缺血性脑血管病CTA与MR T1W-SPACE对比研究

【摘要】 目的通过与CT血管造影(CTA)比较,评价MR T1W-SPACE血管成像对缺血性脑血管病的临床诊断价值。方法选取2014年1月至2015年10月深圳福田区人民医院神经内科收治的50例缺血性脑血管病患者,所有患者均行CTA检查,同时一周内行MR T1W-SPACE检查,比较CTA与T1W-SPACE对血管的显示情况。结果 50例患者共350条血管纳入研究,CTA检查显示:正常或轻度狭窄162条、中度狭窄92条、重度狭窄96条;T1W-SPACE检查显示:正常或轻度狭窄181条、中度狭窄84条、重度狭窄85条;两组比较差异均无统计学意义(P>0.05)。CTA发现斑块153处,T1W-SPACE发现斑块162处,CTA及T1W-SPACE均可观察到动脉壁,CTA可以对斑块进行大致分型,T1W-SPACE可以对斑块从病理角度分型。结论 CTA图像形象直观,扫描速度快但有辐射,MR T1W-SPACE无辐射,对血管狭窄诊断精准,对斑块观察更具体,可以作为一种新的脑血管成像技术应用于临床。

【Abstract】 Objective To evaluate the clinical diagnostic value of MR T1W-SPACE on ischemic cerebrovascular disease through comparison with CT angiography(CTA). Methods Fifty patients with ischemic cerebrovascular disease in Department of Neurology in Shenzhen Futian People’s Hospital were selected From Januay 2014 to October2015. All patients underwent CTA examination, and received MR T1W-SPACE examination within one week. The display of blood vessels by CTA and T1W-SPACE were compared. Results A total of 350 vessels in fifty patients were included in the study. CTA examination showed normal or mildly stenosis in 162 vessels, moderate stenosis in 92 vessels,severe stenosis in 96 vessels, and T1W-SPACE examination showed normal or mildly stenosis in 181 vessels, moderate stenosis in 84 vessels, severe stenosis in 85 vessels, with no significant difference between the two groups(P>0.05). A total of 153 plaques were found in CTA, and T1W-SPACE found 162 plaques. Arterial wall was observed in both CTA and T1W-SPACE. CTA could classify the plaques generally into calcified plaque, soft plaque and mixed plaque, while T1W-SPACE contributed to the pathological classification. Conclusion CT scanning is direct and fast, but the radiation can not be ignored. T1W-SPACE brings no radiation and results in more accurate observation of intracranial vascular stenosis and more detailed observation of the plaque, which can be used as a new intracranial vascular imaging technique in clinical application.
