辅助T细胞17及其相关调控因子在特应性皮炎患者中的表达情况及其临床意义

【摘要】 目的探讨辅助T细胞17(Th17)及其相关调控因子白细胞介素-23(IL-23)和微小RNA155(miR-155)在特应性皮炎(AD)患者外周血中的表达情况及其临床意义。方法选择54例AD患者和30例健康对照者,检测受试者外周血白细胞中Th17比例、IL-23、miR-155的表达水平,分析3者诊断AD的敏感性和特异性以及3者与AD严重程度特应性皮炎评分(SCORAD)的相关性。结果 AD患者Th17比例、IL-23和miR-155表达水平均显著高于健康对照者(P<0.05);Th17比例、IL-23和miR-155诊断AD的敏感性分别77.64%、65.30%和82.19%,特异性分别为62.81%、74.28%和77.06%;Th17比例、IL-23和miR-155表达水平均与SCORAD评分呈正相关(P<0.05)。结论 IL-23和miR-155可能参与了AD中Th17细胞的表达调控,Th17细胞及其相关因子尚不能作为AD的诊断指标,但可以辅助判断其疾病严重程度。

【Abstract】 Objective To explore the expression and clinical significance of T helper cell 17( Th17) and its related regulatory control factors including interleukin-23( IL-23) and microRNA-155( miR-155) in peripheral blood of the patients with atopic dermatitis( AD). Methods A total of 54 patients with AD and 30 healthy individuals were enrolled. Then the proportion of Th17 in peripheral leukocytes,and the expression levels of IL-23 and miR-155 in peripheral blood were detected in all participants. The sensitivity and specificity of the three factors for the diagnosis of AD were analyzed. And the correlation of the three factors with AD severity[scoring atopic dermatitis index( SCORAD) ]was assessed. Results The proportion of Th17,and the expression levels of IL-23 and miR-155 of AD patients were significantly higher than those of healthy individuals( P < 0. 05). The sensitivities of proportion of Th17,IL-23 and miR-155 for the diagnosis of AD were77. 64%,65. 30% and 82. 19%,respectively,while the specificities were 62. 81%,74. 28% and 77. 06%,respectively. The proportion of Th17,IL-23 and miR-155 expression levels positively correlated with SCORAD score( P < 0. 05). Conclusion IL-23 and miR-155 may be involved in regulating the expression of Th17 cell in AD. Th17 cell and its related regulatory factors still can not act as the diagnostic indicator for AD,but they are helpful for evaluating the severity of disease.
