【摘要】 目的探讨腹腔镜病灶切除术后亮丙瑞林联合曼月乐后续治疗子宫腺肌病的临床价值。方法以我院妇科门诊2014年8月~2015年8月收治的子宫腺肌病患者共104例为对象。按照患者入院顺序号,采用随机数字表法分为观察组和对照组各52例。行腹腔镜病灶切除术后,对照组采用亮丙瑞林治疗。观察组采用亮丙瑞林联合曼月乐治疗。两组患者均随访1年。比较两组治疗前、治疗3、6、12个月后的月经量PBAC评分和痛经VRS评分,并记录两组治疗后的不良反应。结果观察组治疗3、6和12个月后的月经量PBAC评分分别为(41.15±6.61)分、(33.38±4.11)分、(23.62±2.93)分,与同时期对照组评分差异有统计学意义(t=6.172、4.836、4.365,P<0.05)。观察组治疗3、6和12个月后的痛经VRS评分分别为(1.32±0.24)分、(0.98±0.17)分、(0.46±0.15)分,与同时期对照组评分差异有统计学意义(t=3.985、4.012、4.996,P<0.05)。两组治疗后更年期症状、阴道不规则流血、骨痛、乳房肿痛以及其他不良反应发生率差异不具有统计学意义(χ~2=0.078、0.102、0.000、0.343、0.443,P>0.05)。结论相比于病灶切除术后单纯运用亮丙瑞林治疗子宫腺肌病,亮丙瑞林联合曼月乐治疗能够获得更好的效果。更多还原

【Abstract】 Objective To investigate the curative effect of leuprorelin combined with Mirena in the treatment of adenomyosis after laparoscopic resection. Methods 104 patients with adenomyosis treated in our hospital from August 2014 to August 2015 were divided into observation group(n=52) and control group(n=52) according to the number of patients admitted to hospital by the random number table.The observation group was treated by leuprorelin combined with Mirena after laparoscopic resection while the control group was treated by the leuprorelin treatment.Two groups were followed up for 1 year to compare the menstrual volume PBAC score and dysmenorrhea VRS score before treatment,3 months,6 months,12 months after treatment,and record the adverse reactions of the two groups after treatment. Results The PBAC scores in the observation group were respectively(41.15+6.61),(33.38+4.11),(23.62+2.93) points 3 months,6 months,12 months after treatment,which were statistically higher than those of the control group(t=6.172, 4.836, 4.365,P < 0.05).The VAS scores in the observation group were respectively(1.32+0.24),(0.98+0.17),(0.46+0.15) points 3 months,6 months,12 months after treatment,which were statistically lower than those of the control group(t=3.985,4.012,4.996,P < 0.05).The menopausal symptoms,irregular vaginal bleeding,pain,breast pain and other adverse reactions of two groups after treatment were not significant difference(χ2=0.078,0.102,0,0.343,0.443, P > 0.05). Conclusion Leuprorelin combined with Mirena has better effect in the treatment of the adenomyosis after lesion resection compared treated by simply using leuprolide.更多还原
