【摘要】 目的分析威海地区某二甲医院干预前后普外科围手术期预防性应用抗菌药物情况,评价干预效果。方法选取威海地区某二甲医院普外科2015年3月—5月(干预前)与2016年3月—5月(干预后)的围手术期病例各122例,对比分析干预前后抗菌药物预防性应用情况。结果在用药指征、用药疗程、药物选择、用法用量、无指征换药、用药途径等方面干预后用药的不合理情况明显少于干预前;干预前用药指征略高于干预后,且用药指征多存在于Ⅱ类切口;干预前用药头孢菌素类占49%,喹诺酮类占29%,干预后头孢菌素类占79%,喹诺酮类占13%;干预后患者的治疗疗程明显缩短,感染率降低,费用明显减少。结论积极的干预普外科围术期抗菌药物预防用药,可改善其不合理用药情况,降低患者术后感染的发病率、减少住院时间和费用。更多还原

【Abstract】 Objective To analyze the prophylactic use of antibiotics during perioperative period in general surgery and to evaluate the efficacy of the intervention. Methods 122 perioperative cases of general surgeries from March to May 2015(before the intervention) and March to May 2016(after the intervention)in the dimethyl hospital in Weihai were respectively selected and analyzed. Result After the intervention, there was a significant decline in irrational use of drug in terms of medical indications, drug treatment, drug selection, usage and dosage, no indications treatment and medication way than before. The medicine indications was slight higher before the intervention, which always happen toⅡ class incision. The proportion of usage of cephalosporin class raised from 49% to 79% and azole nuo ketone accounted from 29% to 13% after the intervention. Compared with before the intervention, there was a dramatically downward trend in patients’ treating duration, infection rate and treatment costs after the intervention. Conclusion Active intervention in general surgery perioperative by antimicrobial prophylaxis can improve the irrational medication, decrease the incidence of postoperative infection and reduce hospitalization time and cost.更多还原
