细胞增殖核抗原Ki-67、血管内皮生长因子与缺氧诱导因子-1a在结直肠腺癌组织中表达的临床意义
Clinical significance expression of cellular proliferative nuclear antigen Ki-67,vascular endothelial growth factor and hypoxia-inducible factor-1a in colorectal adenocarcinoma tissue
【摘要】 目的观察细胞增殖核抗原Ki-67、血管内皮生长因子(VEGF)和缺氧诱导因子-1a(HIF-1a)蛋白在结直肠腺癌组织中的表达。方法用免疫组化SP法检测32例结直肠腺癌组织中Ki-67、VEGF和HIF-1a蛋白的表达。结果在结直肠腺癌组织中,HIF-1a和VEGF蛋白阳性表达率分别为59.38%(19/32例)和65.63%(21/32例),Ki-67蛋白的阳性表达率为75.0%(24/32例),与癌旁组织比较差异有统计学意义(P<0.01,P<0.05,P<0.05)。Ki-67、VEGF和HIF-1a蛋白表达与结直肠腺癌国际通用的TNM(T:肿瘤大小及局部浸润范围,N:淋巴结受累情况,M:远处转移)分期法中的分期、浸润深度密切相关;而与性别、年龄、肿瘤直径和分化程度无关。Ki-67蛋白表达在浸润深度T3+T4比例为91.30%(21/23例),较浸润深度T1+T2比例33.33%(3/9例),差异有统计学意义(P<0.05);Ki-67蛋白表达在TNM分期Ⅲ+Ⅳ比例为100%(12/12例),较Ⅰ+Ⅱ比例60%(12/20例)升高但差异无统计学意义(P>0.05)。VEGF蛋白表达在浸润深度T3+T4比例为82.60%(19/23例),较浸润深度T1+T2比例22.22%(2/9例)明显升高,差异有统计学意义(P<0.05);VEGF蛋白表达在TNM分期Ⅲ+Ⅳ比例为91.67%(11/12例),较Ⅰ+Ⅱ比例50%(10/20例)明显升高,差异有统计学意义(P<0.05)。HIF-1a蛋白表达在浸润深度T3+T4比例为73.91%(17/23例),较浸润深度T1+T2比例22.22%(2/9例)明显升高,差异有统计学意义(P<0.05);HIF-1a蛋白表达在TNM分期Ⅲ+Ⅳ比例为91.67%(11/12例),较Ⅰ+Ⅱ比例40%(8/20例)明显升高,差异有统计学意义(P<0.05)。癌组织中的Ki-67、VEGF和HIF-1a蛋白表达,三者呈正相关,差异有统计学意义(P<0.05)。结论 Ki-67、VEGF和HIF-1a蛋白在癌组织中表达,其可能与结直肠腺癌的发生、发展及淋巴结转移有关。
【Abstract】 Objective To observe the expression of Ki-67,the vascular endothelial growth factor(VEGF) and hypoxia inducing factor-1a(HIF-1a) protein in the tissue of colorectal adenocarcinoma.Methods The expression of Ki – 67,VEGF and HIF-1a protein was determined in 32 cases of colorectal adenocarcinoma tissue by immunohistochemical SP method.Results The expression rate of protein positive of HIF-1a and VEGF in colorectal adenocarcinoma tissues were59.38%(19/32) and 65.63%(21/32).The expression rate of proteinpositive of Ki-67 protein positive expression rate was 75.0%(24/32).Compared with the tissue adjacent to carcinoma were significant difference in HIF-1a,VEGF and Ki-67(P < 0.01,P < 0.05,P < 0.05).The expression of Ki-67,VEGF and HIF-1 a protein is closely related to colorectal adenocarcinoma TNM staging,infiltration depth(all P< 0.05).While with gender,age,tumor diameter has nothing to do with the degree of differentiation.The ratio of Ki-67 protein expression in infiltrating depth T3 + T4 was 91.30%(21/23 cases),the difference was significant compared with the ratio of 33.33%(3/9 cases) in invasion depth T1 + T2(P < 0.05).But the ratio of expression in TNM stage Ⅲ + Ⅳ was 100%(12/12 cases),the difference was not significant compared with the ratio of 60%(12/20 cases) in Ⅰ + Ⅱ(P > 0.05).The ratio of VEGF protein expression in infiltrating depth T3 + T4 was 82.60%(19/23cases),the difference was significant compared with the ratio of 22.22%(2/9 cases) in invasion depth T1 + T2(P <0.05).And the ratio of expression in TNM(T:score is based upon the size and/or extent of invasion;N:score indicates the extent of lymph node involvement;M:score indicates whether distant metastases are present) stage Ⅲ + Ⅳ was91.67%(11/12 cases),the difference was significant compared with the ratio of 50%(10/20 cases) in stage Ⅰ + Ⅱ(P < 0.05).The ratio of HIF-1a protein expression in infiltrating depth T3 + T4 was 73.91%(17/23 cases),the difference was significant compared with the ratio of 22.22%(2/9) in invasion depth T1 + T2(P < 0.05).And the ratio of expression in TNM stage Ⅲ + Ⅳ was 91.67%(11/12 cases),the difference was also significant compared with the ratio of 40%(8/20) in stage Ⅰ + Ⅱ(P < 0.05).The protein expression of Ki-67,VEGF and HIF-1a in carcinoma tissues were positively correlated,the difference was significant(P < 0.05).Conclusion The protein expression of Ki-67,VEGF and HIF-1 in cancer tissue may be related to the occurrence and development of colorectal adenocarcinoma and lymph node metastasis.
