【摘要】 目的采用系统评价的方法对单束重建与双束重建治疗前交叉韧带断裂进行疗效评价。方法本研究利用系统评价的方法 ,系统、全面收集并且定量分析自1948-11—2015-01发表的有关单束与双束重建治疗前交叉韧带断裂的随机对照研究。本系统评价一共纳入17篇随机对照研究,共1 394例,其中接受双束重建的有685例,接受单束重建的有808例。结果纳入研究中,有15篇研究报道了KT-1000或KT-2000,结果显示双束前交叉韧带重建术更有效恢复膝关节前后稳定性[WMD:-0.42,95%CI(-0.65,-0.20),P<0.001];有14篇研究报道了轴移试验,结果显示双束重建更有效恢复膝关节旋转稳定性[RR:1.32,95%CI(1.15,1.52),P<0.001]。而对于膝关节功能Lysholm评分,2组之间比较差异无统计学意义[WMD:0.51,95%CI(-0.61,1.63),P=0.370]。结论与单束重建相比,双束前交叉韧带重建能更有效恢复膝关节稳定性。更多还原

【Abstract】 Objective To evaluate the efficacy of double-bundle versus single-bundle reconstruction for the treatment of anterior cruciate ligament(ACL) disruption by a Meta-analysis. Methods The randomized controlled trials that compared double-bundle and single-bundle reconstruction for the treatment of anterior cruciate ligament(ACL) disruption from Nov.1948 to Jan. 2015 were systemically reviewed and quantitatively analyzed. Altogether, 17 trials with a number of 1 394patients(685 in DB group and 808 in SB group) contributed data were included in our meta-analysis. Results In trials that reported KT-1000 or KT-2000 as an outcome(15 trials, n =1 341), double-bundle reconstruction was associated with-0.42 mm improvement in anterior stability(95% CI:-0.65 to-0.20, P <0.001) through the single-bundle approach. In trials that reported pivot-shift test as an outcome(14 trials, n =1 304), double-bundle reconstruction was associated with superior capacity for rotational stability with the pooled relative risk value(RR) of 1.32(95%CI: 1.15 to 1.52, P <0.001). However, when coming to clinical measure of Lysholm score, there were no significant differences between double-bundle and single-bundle reconstruction at final follow-up(weighted mean difference of 0.51, 95% CI:-0.61 to 1.63, P =0.370). Conclusion Compared with single-bundle reconstruction, double-bundle reconstruction can better restore stability of knee.更多还原
