布地奈德对成年过敏性鼻炎患者调节性T细胞功能的影响

【摘要】 目的观察布地奈德对成年过敏性鼻炎(AR)患者CD8~+CD28~-调节性T细胞(Treg)功能的影响。方法 AR患者70例,随机分为观察组及对照组,每组35例,均以孟鲁斯特作为基础治疗,观察组给予布地奈德鼻喷雾剂128μg/(鼻孔·天),对照组不给予布地奈德喷鼻。另选择32例健康志愿者作为健康对照组。检测健康对照组及AR患者治疗前及治疗12周后的外周血Treg及其胞内细胞因子白介素10(IL-10)、转化生长因子β1(TGF-β1)的含量,以及IgE、IgG4和嗜酸性粒细胞(EOS)含量。结果(1)与健康对照组比较,AR患者CD8~+CD28~-Treg、IL-10、TGF-β1及IgG4均显著降低(P<0.01),而IgE及EOS均显著升高(P<0.01)。(2)治疗后观察组CD8~+CD28~-Treg、IL-10、TGF-β1及IgG4均高于治疗前及对照组(P<0.01),IgE及EOS均低于治疗前及对照组(P<0.01或0.05)。(3)CD8~+CD28~-Treg、IL-10及TGF-β1三者当中任意一者,均与IgE及EOS当中任一者呈负相关(P<0.01或0.05),但前三者与IgG4均呈正相关(P<0.05或0.01)。结论 CD8~+CD28~-Treg及其细胞因子下降是AR的重要表现,布地奈德可提高该细胞及IgG4含量,降低IgE及EOS含量。

【Abstract】 Objective To observe the effect of budesonide on CD8~+CD28~- regulatory T cells in adult patients with allergic rhinitis.Methods Seventy patients with allergic rhinitis were randomly divided into Control Group and Observation Group,35 cases in each group.The two groups received montelukast as basic treatment,and Observation Group was administered with 128 μg budesonide nasal spray(nostril-day).32 healthy volunteers were selected as Healthy Control Group.CD8~+CD28~- Treg ratio in PBMCs,IL-10 and TGF-pi were detected in Healthy Control Group and the patients with allergic rhinitis by Flow cell technology before treatment and 12 weeks after treatment,and IgE,IgG4 and EOS were also monitored with ELISA.Results(1) Compared with Healthy Control Group,CD8~+CD28~- Treg ratio,IL-10,TGF-(31 and IgG4 in the patients with allergic rhinitis were significantly lower(P<0.01),while the IgE and EOS were all significantly increased(P<0.01).(2) After the treatment,CD8~+CD28~- Treg,IL-10,TGF-β1 and IgG4 in Observation Group were significantly higher than those before the treatment and of Control Group(P<0.01 or 0.05).(3) CD8~+CD28~- Treg,IL-10 and TGF-(31 respectively was negatively correlated with IgE or EOS and showed a positive correlation with IgG4(P<0.01 or 0.05).Conclusion Budesonide can reduce inflammation in allergic rhinitis which may be associated with the reduction of CD8~+CD28~- Treg cells and its cytokines.
