盆底表面肌电生物反馈疗法治疗功能性便秘的临床效果评估

【摘要】 为探讨盆底表面肌电生物反馈疗法治疗功能性便秘的临床效果,对符合纳入标准的120例功能性便秘患者给予盆底表面肌电生物反馈疗法治疗,每次30 min,每周2次,10次为1个疗程,2个疗程后行凯格尔盆底肌肉家庭锻炼,治疗1个月后从临床症状、生活质量及盆底表面肌电Glazer评估方面评价疗效。结果显示,本组16例患者因未能坚持治疗退出试验,最终104例纳入统计。104例患者中,显效4例,有效45例,无效55例,总有效率为47.12%(49/104)。与治疗前相比,治疗后排便不尽感、排便费力、排便时间方面评分及症状总积分均明显降低,P<0.01;排便次数、大便性状方面评分无明显变化,P>0.05。治疗后患者生活质量各因子评分及总评分均明显降低,P<0.01。盆底表面肌电Glazer评估发现,治疗前后前基线平均波幅、最大收缩力、平均收缩力及变异系数差异有统计学意义,P<0.01或P<0.05;10 s放松、休息放松及后基线平均波幅比较差异无统计学意义,P>0.05。结果表明,盆底表面肌电生物反馈可以显著改善功能性便秘患者的临床症状、心理状态及患者盆底肌的收缩功能。

【Abstract】 In order to explore the clinical effect of pelvic floor electromyography(EMG) biofeedback therapy in the treatment of functional constipation,120 patients conformed to inclusive criteria with functional constipation were treated with pelvic floor EMG biofeedback therapy for 30 min,2 times a week,10 times as a course of treatment;after 2 courses performed Kegel pelvic floor muscle home exercise;after 1 month of treatment evaluated the efficacy from the clinical symptoms,quality of life and pelvic floor surface electromyography Glazer assessment.As results,because of failure to adhere to treatment 16 patients in this group were quit the test;Finally,104 cases were included in the statistics.In the 104 patients,4 were excellent,45 were effective,55 ineffective;the total effective rate was 47.12%(49/104).The scores on unfinished defecation feeling,defecation difficulties,defecation time and their whole scores after treatment were all significantly lower than those before treatment(P<0.01).But there were no obvious changes in defecation frequency and stool quality(P>0.05).After treatment,the score on quality of life factor and the total score were significantly decreased(P<0.01).The pelvic floor surface electromyography Glazer assessment found there was statistical significance in the mean baseline amplitude,maximal contraction force,mean contraction force and coefficient of variation before and after treatment(P<0.01 or P<0.05);in 10 s relaxation,rest relaxation and the mean baseline amplitude there was no statistically significance(P >0.05).The results show that the pelvic floor EMG biofeedback can significantly improve the clinical symptoms,psychological state and pelvic floor muscle contraction of patients with functional constipation.
