HPVL1蛋白和HMGA1在子宫颈病变中的表达及临床意义

【摘要】 目的人乳头状瘤病毒(HPV)L1壳蛋白及高迁移率家族蛋白(HMGA1)在不同宫颈病变中的表达及相关性。方法选取经病理证实至少2年同一型别高危HPV阳性的低级别鳞状上皮内病变(LSIL)34例、高级别鳞状上皮内病变(HSIL)42例及宫颈鳞状细胞癌(SCC)5例为研究组,慢性宫颈炎20例为对照组。采用非同位素核酸分子杂交与免疫细胞化学技术测定HPV L1蛋白的表达,免疫组化法测定HMGA1的表达。结果 101例患者中,HPV L1阳性表达率为31.68%(32/101),对照组、LSIL、HSIL和SCC HPV L1蛋白阳性表达率分别为60.00%(12/20)、44.12%(15/34)、11.90%(5/42)和0(0/5),其中对照组与HSIL和SCC组比较,LSIL组与HSIL和SCC组比较,差异均有统计学意义(P<0.05),其他宫颈病变组比较,差异均无统计学意义(P>0.05);HGMA1的阳性表达率为43.56%(44/101),对照组、LSIL、HSIL及SCC组的阳性率分别为20.00%(4/20)、20.59%(7/34)、66.67%(28/42)和5/5。其中对照组与HSIL和SCC组比较,LSIL组与HSIL和SCC组比较,差异均有统计学意义(P<0.05),其他宫颈病变组比较,差异均无统计学意义(P>0.05)。结论随着宫颈病变的进展,HPV L1壳蛋白阳性表达率而呈降低趋势,而HMGA1蛋白阳性表达率呈升高趋势,对子宫上皮内瘤变的检测可能具有重要意义。

【Abstract】 Objective To explore the expression and correlation of the human papilloma virus L1(HPV L1)capsid protein and high mobility group protein(HMGA1)in the various cervical lesions. Methods 101 patients with HPV infection and cervical lesions confirmed by histopathological diagnosed in Beijing Chuiyangliu Hospital from December 2013 to December 20l5 recruited in this study. There were 20 cases of chronic cervicitis, 76 cases of cervical intraepithelial neoplasia(LSIL level of 34 cases,HSIL level of 42 cases) and in 5 cases of invasive squamous cell carcinomas(SCC). Nucleic acid molecule hybridization and immunocytochemistry technique was used to detected HPV Ll capsid protein and immunohistochemical staining was performed for HMGA1 expression. Results The positive expression rate of HPV L1 was 31.68% in these 101 patients, and was 60.00%(12/20) in chronic cervicitis,44.12%(15/34) in LSIL,11.90%(5/42) in HSIL and 0(0/5) in SCC. The positive rate of HPV L1 in LSIL, HSIL and SCC, had significant difference compare to chronic cervicitis(P<0.05). The positive expression rate of HMGA1 was 43.56% in the 101 patients, 20.00%(4/20), 20.59%(7/34), 66.67%(28/42), and 5/5 in chronic cervicitis, HSIL and SCC group compared to chronic cervicitis, the positive rate of HMGA1 in LSIL、HSIL and SCC, had statistically difference(P<0.05). Conclusions The positive expression rate of HPVL1 capsid protein is decreasing with the evolution and development of cervical lesions while the positive expression rate of HMGA1 protein is increasing with the evolution and development of cervical lesions. They may be a maker to the predict the biological behavior of precancerous lesions.
