【摘要】 目的研究经鼻高流量湿化氧吸入疗法(HHFNC)对急性低氧性呼吸衰竭的疗效。方法选择该院重症医学科2015年4月至2015年10月收治的41例行氧吸入疗法治疗的急性低氧性呼吸衰竭患者。随机分成面罩法(FM)组(n=10)、HHFNC组(n=15)和无创正压通气(NPPV)组(n=16)进行治疗。分别监测治疗前与治疗2h后呼吸频率、血气分析、血流动力学指标及评价口腔干燥度和舒适度状况。结果 3组治疗2h后PaO2均显著高于治疗前(P<0.05)。治疗2h后NPPV组PaO2显著高于HHFNC组和FM组(P<0.05),并且HHFNC组显著高于FM组(P<0.05)。FM组口腔干燥度显著高于NPPV组和HHFNC组(P<0.05)。HHFNC组舒适度显著高于NPPV组和FM组(P<0.05),并且治疗2h后显著高于治疗前(P<0.05)。结论 HHFNC是治疗急性低氧性呼吸衰竭的一种比较理想的方法。更多还原

【Abstract】 Objective to investigate the effect of humidified high flow nasal cannula oxygen therapy(HHFNC)to acute hypoxic respiratory failure.Methods 41 patients with acute hypoxic respiratory failure were enrolled who entered ICU from April,2015 to October,2015 and randomly divided into face mask(FM)group(n=10),HHFNC group(n=15)and noninvasive positive pressure ventilation(NPPV)group(n=16).Respiratory rate,blood gas analysis,hemodynamic data and parameters in relation to subject comfort and humidity within mouth were recorded as based level and 2h after therapy.Results 2hafter therapy,all groups had significant increase in PaO2 compared to base line(P<0.05).Meanwhile,NPPV group had higher level of PaO2 in comparison to HHFNC and FM group(P<0.05),similarity,PaO2 of HHFNC group was higher than the FM group.In this study,the humidity within mouth of FM group was higher than the NPPV group and HHFNC group(P<0.05).The subject feeling of comfort in HHFNC group was significantly higher than NPPV group and FM group(P<0.05).Conclusion HHFNC is an alternative strategy for acute hypoxic respiratory failure which helps to correct hypoxemia and improve subject feeling of comfort.更多还原
