中药复聪汤拮抗庆大霉素耳中毒的研究

【摘要】 目的:用纯中药制剂复聪汤拮抗庆大霉素(GM)耳中毒性,观察对耳蜗毛细胞和听神经的保护效果。方法:选用60只健康青年豚鼠,分成GM拮抗组,GM对照组和正常对照组,每组20只。GM拮抗组动物给予GM连续肌肉注射的同时口服纯中药制剂复聪汤拮抗连续30d,GM对照组动物仅GM连续肌肉注射30d。分别在治疗30、60、90d后测定动物的听功能(ABR,DPOAE)后处死6只动物,耳蜗行扫描电镜和光镜观察及毛细胞计数。结果:在连续30d的GM注射的GM拮抗组和GM对照组ABR平均阈值上升;GM拮抗组在中药拮抗30d后,豚鼠的ABR反应阈值明显上升,DPOAE幅值有较明显降低;耳蜗铺片显示耳蜗外毛细胞(OHC)的存活数目明显下降,扫描电镜观察到耳蜗OHC大量损害消失,静纤毛束折断。在拮抗治疗60d后,耳蜗毛细胞数目明显增多;GM拮抗组到90d后的耳蜗毛细胞形态整体基本完好,并可见到支持细胞转分化为新生毛细胞现象,且听功能已基本恢复正常。结论:在注射GM的同时给予中药进行拮抗治疗,反而加重了耳蜗毛细胞的损害,而在后期的连续中药治疗可以在一定程度上促进耳蜗受损毛细胞的修复和听功能的改善。

【Abstract】 Objective:Cochlea hair cells damaged and lost is leading cause of permanent the sensorineural deafness.To study effectiveness of traditional Chinese medicine(TCM) in antagonst effects damage of gentamicin-induced hair cells loss.Methods:Sixty adult guinea pigs were used in this experiment.Auditory function test of auditory brainstem response(ABR) and distortion product otoacoustic emission(DPOAE) was performed on the individual animals which was divided three groups:20 gentamicin(GM)damaged control group,20 GM damaged treating group,which two groups animals were intrperitoneally injected with GM 80mg/kg per day for consecutive 30 d.Anther 20 guinea pigs were used as normal control group.GM damaged treating group were treating with Fucong Mixture Liquid for consecutive 30 d,same volume of physiological saline was given to GM damaged control group and normal control group in the same time.At the tearting 30,60 and 90 d time,the experment animals’ ABR and DPOAE were assess the variation of auditory function in each group.In this time,6 experment animals were killing after auditory function assess,Cochlear surface preparation,the number of cochlea outer hair cells were counted under the light microscope and the morphological changes of cochlea were observed under the scanning electronic microscope(SEM).Results:Under GM was injected 30 d after,the ABR threshold were increased than not give GM group,as well as the DPOAE amplitudes value was decreased significantly in the GM damaged treating group,especially in the high-frequency areas that correlated with the greatest hair cells loss to the base of cochlea.Light and SEM showed that ototoxic insult greatest hair cells might loss and hair bundle might fragment and stereocilia might become splayed.At the antagonist tearting 30 d of TCM,there was not recovery of the ABR and DPOAE function.At the antagonist treating 60 d of TCM,the ABR and DPOAE function was significantly recovery,as well as the hair cells numbers was increst.And at the treating 90 d of TCM,more important found that was appeared norme cochlea,and that to can be seen some of the new hair cells appear cochlear supporting cells can directly transdifferentiate into OHCs after aminoglycoside exposure.Conclusion:This expermental suggest thatTCM treatment group not have a antagonist effects on the cochlea hair cells injury by gentamycin,we found that using TCM treating can be promote hair cells and auditory function recovery of ototoxic after.
