【摘要】 目的探讨乌司他丁联合早期肠内营养治疗重症急性胰腺炎的临床效果及对患者肠粘膜屏障的影响。方法将92例重症急性胰腺炎患者随机分为两组,各46例。联合组给予乌司他丁联合早期肠内营养治疗,对照组仅采用早期肠内营养治疗。疗程均为14 d,对比两组治疗效果及对患者肠粘膜屏障的影响。结果治疗14 d后,联合组治疗效果高于对照组(P<0.01),血淀粉酶复常、腹部缓解、肠鸣音恢复及腹部压痛缓解时间均短于对照组(P<0.01);两组治疗后APACHE II评分、血清D-乳酸和二胺氧化酶均降低(P<0.01),且联合组降低幅度均大于对照组(P<0.01)。结论乌司他丁联合早期肠内营养治疗重症急性胰腺炎效果显著,可明显改善患者肠粘膜屏障。更多还原

【Abstract】 Objective To investigate efficacy of Ulinastatin( UTI) combined with early enteral nutrition therapy in patients with severe acute pancreatitis and its effect on intestinal mucosa barrier. Methods Ninety-one patients with severe acute pancreatitis patients admitted to hospital were randomly divided into combined group( n = 46) and control group( n = 45). Combined group underwent ulinastatin combined with early enteral nutrition therapy,while the control group underwent only early enteral nutrition therapy,and the treatment duration were 14 d. Results Total efficiency of 14 d of the combined group was better than than of control group( P <0. 01); serum amylase normalization time,relieve abdominal time,recovery time of bowel sounds and abdominal tenderness remission time of the combined group were shorter than those of the control group( P < 0. 01); APACHE II score decreased with statistical significance( P < 0. 01); and APACHE II scores of combined group were lower than that of the control group( P < 0. 01); serum levels of Dlactic acid and diamine oxidase decreased( P < 0. 01); joint serum D-lactic acid and diamine oxidase of combined group were lower than those of the control group,with statistical significance( P < 0. 01).
