北京市丰台社区脑卒中高危人群颈动脉筛查结果分析
Analysis of the investigation of carotid atherosclerosis in high-risk population of stroke in Beijing Fengtai community
【摘要】 目的探讨影响脑卒中高危人群颈动脉硬化及斑块形成的相关危险因素。方法对3215例脑卒中高危人群进行颈动脉B超筛查,根据内膜是否增厚及斑块数量进行分组,比较各组内膜是否增厚、斑块数量与高危因素间的相互关系。结果脑卒中高危人群中有内膜增厚组与无内膜增厚组在性别、血脂异常、糖尿病病史方面差异有统计学意义(P<0.05);斑块数量不同组人群在性别、吸烟与否、超重、卒中家族史差异有统计学意义(P<0.05);多因素Logistic回归分析显示,颈动脉内膜增厚与否与血脂异常、糖尿病明显相关;内膜增厚与否在虚证患者中差异有统计学意义(P<0.05)。结论血脂异常、糖尿病可能是颈动脉内膜增厚的独立危险因素;斑块形成的多少受吸烟、超重、卒中家族史影响更明显。在有颈动脉内膜增厚患者中虚证较实证更多见。
【Abstract】 Objective To investigate the correlation of carotid artery plaque and other risk factors in high-risk stroke population. Methods 3215 cases of high-risk population of stroke were investigated by carotid artery ultrasound and then divided into the different groups by carotid intima-media thickening,the number of plaque,and then the interrelations of risk factors among the groups were compared. Results There were significantly statistical differences in gender,dyslipidemia,and diabetes history between the intimal thickening group and non intimal thickening group in high-risk stroke population( P < 0. 05). There were significantly statistical differences in the number of plaques between the different groups in the sex,smoking or not,overweight,family history of stroke( P < 0. 05). Logistic multi factor regression analysis showed that dyslipidemia and diabetes mellitus were associated with intima media thickness( IMT) of carotid artery. There was statistical significance in patients with deficiency syndrome in difference of intimal thickening( P < 0. 05). Conclusion Dyslipidemia and diabetes may be the independent risk factors for carotid intimal thickening,and the formation of plaque is more significantly influenced by smoking,overweight,and family history of stroke. In patients with carotid artery intimal thickening,deficiency syndrome is more seen than the excess syndrome.
