血浆miR-193b和miR-199a在食管癌诊治中的临床价值

【摘要】 目的探讨miRNA-193b和miRNA-199a在血浆中的表达对食管鳞癌的诊治价值。方法选取2014年9月至2015年6月之间食管鳞癌患者56例,同期健康体检者30例。采用qRT-PCR技术(Taq Man探针法)检测食管鳞癌患者和健康者血浆中miRNA-193b、miRNA-199a的表达水平,以及检测食管鳞癌患者术前、术后1w血浆中miRNA-193b、miRNA-199a的表达水平,并结合临床病理资料,探讨它们与食管鳞癌的关系。结果食管鳞癌患者血浆中miR-193b和miR-199a的表达量均明显低于健康体检者(P<0.05);食管癌患者术后血浆中miR-193b和miR-199a的表达量均明显高于术前(P<0.05);血浆中miR-193b的表达与肿瘤TNM分期有关(t=-5.332,P=0.000),表现为随着TNM分期越晚,表达逐渐下调的趋势,与患者年龄、性别、肿瘤分化程度、淋巴结转移无明显关联(P>0.05)。miR-199a的表达与肿瘤分化程度、肿瘤TNM分期、淋巴结转移有关(P<0.05),表现为随着分化程度越低、TNM分期越晚和出现淋巴结转移,表达逐渐下调的趋势,而与患者年龄、性别无明显关联(P>0.05)。结论 miRNA-193b、miRNA-199a在食管鳞癌患者血浆中表达下调,可能作为抑癌基因参与食管鳞癌的发生发展。

【Abstract】 Objective To investigate the value of miRNA-193 b and miRNA-199 a in the diagnosis and treatment of esophageal squamous cell carcinoma. Methods 56 cases of esophageal squamous cell carcinoma and 30 cases of healthy physical examination were selected. MiRNA-193 b and miRNA-199 a expression levels in esophageal squamous cell carcinoma patients and healthy persons were detected by qRT-PCR technique,and the expression levels of miRNA-199 a and miRNA-193 b in plasma of patients with esophageal squamous cell carcinoma before and after operation were detected. Results The expression of plasma miR-199 a and miR-193 b in patients with esophageal squamous cell carcinoma was significantly lower than that of healthy controls( P < 0. 05). The expression of plasma miR-199 a and miR-193 b in patients with esophageal carcinoma after operation was significantly higher than that of the preoperative expression( P < 0. 05). The expression of miR-193 b in plasma was related to TNM staging( P = 0. 000,t =- 5. 332),and gradually decreased with the TNM staging. It had no significant association with age,gender,tumor differentiation and lymph node metastasis( P > 0. 05). The expression of miR-199 a was related to the tumor differentiation,TNM stage and lymph node metastasis( P <0. 05),and gradually decreased with the lower differentiation degree,TNM staging,lymph node metastasis. It had no significant association with age and gender of patients( P > 0. 05). Conclusion The expression levels of miRNA-193 b and miRNA-199 a in the plasma of patients with esophageal squamous cell carcinoma is downregulated. It may act as a tumor suppressor gene involved in the development of esophageal squamous cell carcinoma.
