韶关地区低体重早产儿视网膜病筛查3年流行特征及危险因素分析

【摘要】 目的了解韶关地区2011年~2013年低体重早产儿视网膜病(retinopathy of prematurity,ROP)发病趋势和主要影响因素,为制订相关筛查标准、方案提供依据。方法根据《早产儿治疗用氧和视网膜病变防治指南》中的ROP筛查标准,在2011年~2013年期间由专科医生,对韶关地区符合标准的低体重早产儿进行ROP筛查,根据《ROP国际分类法》(2005)进行分期,每周随访一次并记录体重、胎龄、ROP结果。结果 2746份有效ROP筛查数据,阳性共236例(8.6%),重症99例(3.6%),2001年~2013年阳性率和重症率均逐渐降低(χ2=11.172,P=0.025;χ2=14.007,P=0.007);阳性进展至重症的校正胎龄均值为36.5w(95%CI 34.4~40.9w);Logistic回归结果:低胎龄(OR=0.353,95%CI:0.164~0.761)、低出生体重(OR=0.531,95%CI:0.302~0.932)、吸氧时间>5d(OR=2.564,95%CI:1.484~4.430)和出现酸碱平衡状况(OR=4.512,95%CI:1.499~13.582)是ROP的危险因素。结论韶关地区2011年~2013年ROP发病率、重症率呈下降趋势,校正胎龄、出生体重、吸氧时间和酸碱平衡是低体重早产儿ROP的影响因素。

【Abstract】 Objective:To explore retinopathy of prematurity(ROP)situation of low birth weight infant in Shaoguan from 2011~2013,and provide basis for ROP screening standard and program. Method:According to screening standard in oxygen support and ROP prevention of premature guide,low birth weight infant in Shaoguan up to standard received ROP specialized screening from 2011~2013,the ROP patients were phased by International ROP Staging(2005)and follow-up visited with weight,GA,ROP result each week. Result:Total 2746 effective ROP screening data,there were 236 ROP positive newborns(8.6%),and 99 severe ROP(3.6%). ROP positive and severe rate both descended from 2011~2013(χ2=11.172,P=0.025;χ2=14.007,P=0.007). The average CGA when ROP progressed to severe was 36.5w(95%CI 34.4~40.9w). Logistic aggression revealed that low GA(OR = 0.353,95%CI:0.164~0.761),low BW(OR=0.531,95%CI:0.302~0.932)、oxygen support >5d(OR=2.564,95%CI:1.484~4.430)and acid-base imbalance(OR=4.512,95%CI:1.499~13.582)were risky factors of ROP. Conclusion:ROP screening in Shaoguan made some effect. ROP screening should be strenghened for prematurity with BW<1500g,long oxygen support and id-base imbalance when 33~42w CGA.
