职业接触噪声工人听觉脑干反应特点研究

【摘要】 目的探讨噪声性听力损伤的听觉脑干反应(ABR)特点,为职业性噪声聋的诊断提供依据。方法对76例职业接噪工人和19例非接噪工人分别进行中耳分析、耳声发射、ABR检查,结果用SPSS 13.0软件统计分析其相关性。结果 1对照组各波引出率均高于噪声组,对照组Ⅰ~Ⅴ波引出率分别为68.4%~100%,噪声组Ⅰ~Ⅴ波引出率分别为49.3%~86.8%,两组差异均有统计学意义;2噪声组Ⅴ波潜伏期为5.61 ms,对照组Ⅴ波潜伏期为5.42 ms,两组差异有统计学意义;3噪声组不同中、内耳情况Ⅰ、Ⅱ、Ⅲ、Ⅴ波引出率存在差异,差异有统计学意义;4噪声组不同中、内耳情况各波潜伏期及潜伏间期差异无统计学意义。结论 1噪声可引起ABRⅠ~Ⅴ波的单一缺失或联合缺失;2噪声引起潜伏期的延长以Ⅴ波最早出现;3接噪工人中、内耳是否正常对ABR各波引出率有影响,中、内耳越正常,引出率越高,反之越低;4接噪工人中、内耳是否正常对ABR各波的潜伏期及潜伏间期影响不大。

【Abstract】 [Objective]To investigate the auditory brainstem response(ABR)characteristics of workers with noise induced hearing impairment,and provide evidence for diagnosis of occupational noise induced hearing loss. [Methods]A total of 76 workers with occupational exposure of noise and 19 workers without noise exposure received middle ear analysis,acoustic emission,ABR examination,respectively. The result was analyzed by SPSS 13.0 for its correlation. [Results] 1 each wave appearance rate of control group was higher than the noise group,the appearance rate of wave I-V was 68.4%-100% in control group,that in noise group was 49.3%-86.8%,with significance. 2The peak latency of wave V was 5.61 ms in noise group,that in control group was5.42 ms,with significance. 3 In noise group,appearance rate of wave I,II,III,V between different middle and inner ear conditions was significantly different. 4 The peak latency and interpeak latency of ABR was not significantly different between different middle and inner ear conditions. [Conclusion] 1 Noise can cause noise ABR wave I-V single missing or lack of joint.2noise cause the extension of the peak latency for wave V first appeared. 3The normal inner ear or not has effect on the wave appearance rate of noise workers,the more inner ear they have,the higher derivation rate is. 4The inner ear is normal or not has not obvious effect on the peak latency and interpeak latency of ABR.
