【摘要】 目的:探讨超声心动图综合参数在慢性心力衰竭患者诊断及指导治疗中的临床价值。方法:选取2012年1月-2014年6月本院心内科收治的80例慢性心力衰竭患者为研究对象,将80例患者根据随访情况分为随访组(32例)与非随访组(48例)。另选取同期于本院进行体检的健康者30例为正常组。所有研究对象均于入院后24 h内行超声心动图检查及BNP水平等辅助性检查。随访组和非随访组患者入院均进行常规治疗,治疗出院后,随访组定期随访,根据随访情况对患者进行治疗方案的调整。记录三组研究对象的超声心动图综合参数、心衰超声指数及NT-Pro BNP水平。结果:心衰组患者的心衰超声指数与Log NT-Pro BNP水平均高于正常组,两组比较差异有统计学意义(P<0.05)。住院治疗出院时,随访组与非随访组患者在左室收缩功能参数、左室舒张功能参数、心室重构参数、肺动脉收缩压参数、瓣膜返流参数及心衰超声指数方面比较差异无统计学意义(P>0.05);经随访治疗6个月后,随访组患者的左室收缩功能参数、左室舒张功能参数、心室重构参数、肺动脉收缩压参数、瓣膜返流参数及心衰超声指数等均显著低于非随访组,差异有统计学意义(P<0.05)。结论:超声心电图综合参数可用于慢性心力衰竭的诊断,同时可指导治疗,评价预后,可于临床推广应用。更多还原

【Abstract】 Objective:To discuss the clinical value of UCG comprehensive parameters on the treatment of chronic heart failure.Method:80 patients with chronic heart failure from January 2012 to June 2014 in our hospital were selected as research objects and divided into follow-up group for 32 cases and non-follow-up group for 48 cases according the follow up conditions.Another 30 health persons were selected as normal group.All of the subjects were given UCG and BNP examinations within 24 hours.The two groups were given routine treatment when entered in the hospital.After discharge,the follow-up group was followed,with treatment method adjusted based on follow-up results.The UCG comprehensive parameters,heart failure ultrasound parameters,and NT-Pro BNP level of the three groups were recorded.Result:The heart failure ultrasound index and Log NT-Pro BNP level of the heart failure group were higher than the normal group,the differences were statistically significant(P<0.05).When leaving the hospital,there were no significant differences between the two groups in the left ventricular systolic function parameter,left ventricular diastolic function parameter,ventricular remodeling parameter,pulmonary artery systolic pressure parameter,valvular regurgitation parameter,and heart failure ultrasound indicator(P>0.05).After six months of follow-up treatment,the left ventricular systolic function parameters,left ventricular diastolic function parameters,ventricular remodeling parameters,pulmonary artery systolic pressure parameters,valvular regurgitation and heart failure parameters of the follow-up group were significantly lower than the non follow-up group,the differences were statistically significant(P<0.05).Conclusion:UCG comprehensive parameter can be used to diagnose chronic heart failure,guide treatment,evaluate prognosis,which is worth clinical promotion.更多还原
