南平市手足口病死亡病例EV71病毒VP1基因特征分析

【摘要】 目的探讨感染肠道病毒71型(EV71)死亡病例的EV71 VP1基因特征学特征,为公共卫生管理机构及时加强EV71病原学监测提供决策依据。方法收集南平市近年来8例因EV71感染死亡的患儿的标本,进行EV71荧光定量RT-PCR鉴定,采用RT-PCR对6株分离到的EV71进行VP1基因编码区扩增,并对扩增产物进行核苷酸序列测序和分析。根据VP1测序结果与国内外报道的各基因型和基因亚型EV71 VP1序列进行同源性和亲缘进化分析。结果南平市手足口病重症死亡病例的病原体为肠道病毒EV71型C4亚型,与2010年-2012年浙江、江苏及上海流行的为同一亚型,同源性较高。结论 EV71病毒是引起手足口病重症病例的主要病原,通过基因分析发现在153 nt、156 nt处发现了新的点突变类型,在这2个位点出现的碱基T在NCBI数据库中未曾发现,不同位点氨基酸的突变可能成为死亡病例形成的生物学基础。

【Abstract】 Objective To discuss the features of the infectious EV71 VP1 gene on HFMD death cases,in order to provide basis for the public health authorities to strengthen pathogenic surveillance on decision making. Methods Specimens from 8 death cases of EV71 infection in children were collected and EV71 from them were identified by fluorescence quantitative RT- PCR in recent years. VP1 gene coding of 6 isolated EV71 virus were amplified for nucleoside acid sequencing and analysis. Sequence homology and phylogenic evolution analysis were performed according to the sequencing result and reported domestic and foreign genotypes and subtypes of EV71. Results Pathogens from HFMD death cases in Nanping was intestinal virus EV71 C4 subtypes. Compared with the epidemic viruses occurred during 2010- 2012 in Zhejiang,Jiangsu and Shanghai,these viruses belongs to the same subtype. Conclusion EV71 virus is the main pathogen causing HFMD. The new point mutation was found in156 nt and 153 nt by gene analysis. The base T in the NCBI database was not found in the 2 sites. The mutation of amino acid in different sites may be the biological basis of death.
