血培养阳性分离菌及假阳性的报警时间分析
Analysis of the positive alarm time of positive pathogen isolates and false postives in blood culture
【摘要】 目的对本院2014年1月-12月血培养阳性分离菌及假阳性的报警时间进行统计分析。方法采用Bac T/ALERT 3D 120进行血培养,记录并传输阳性瓶报警时间,VITEK-2 Compact对分离菌进行鉴定和药敏试验。采用Excel统计处理数据。结果本院血培养分离的分离菌总体平均阳性报警时间为27.6 h,中位数为19.6 h,94%以上的阳性血培养瓶在72 h内报警;其中平均阳性报警时间和中位数时间最短的为链球菌(12.0 h/11.4 h),最长的为阳性杆菌(42.7 h/40.5 h);阳性报警时间最短(2 h)的为大肠埃希菌、肺炎克雷伯菌、无乳链球菌和黏质沙雷菌,阳性报警时间>120 h的为表皮葡萄球菌(125.5 h)、近平滑念珠菌(130.5 h和131.7 h)和少动鞘氨醇单胞菌(137.1 h)。假阳性血培养的平均阳性报警时间为69.4 h,中位数时间为61.0 h。结论通过对血培养报警时间的统计分析,可对阳性分离菌的种类进行初步预测,并可辅助假阳性的判断。
【Abstract】 Objective To analyze the positive alarm time of positive pathogen isolates and false positive in blood culture during January- December in 2014. Methods Blood specimens were cultured by Bac T / ALERT 3D 120,and the positive alarm time were recorded and uploaded. Pathogens isolated from blood cultures were identified by VITEK- 2 Compact. The data were analyzed by Excel. Results In 2014,the average positive alarm time of all pathogens isolated from blood cultures was 27. 6 h,and the median alarm time was 19. 6 h. 94% of the isolates alarmed within 72 h. The average and median positive alarm time of Streptococcus spp( 12. 0 h /11. 4 h) was the shortest of all species,and Gram positive bacilli( 42. 7 h /40. 5 h) was the longest. The fastest positive alarm time was 2 h,including Escherichia coli,Klebsiella pneumonia,Streptococcus agalactiae and Serratia marcescens. The isolates alarming time more than 120 h included Staphylococcus epidermidis( 125. 5 h),Candida parapsilosis( 130. 5 h and 131. 7 h) and Sphingomonas paucimobilis( 137. 1 h). The average positive alarm time of false positives in blood cultures was 69. 4 h,and the median alarm time was 61. 0 h. Conclusion The positive alarm time can be used to forecast the species of pathogens isolated from blood cultures,and it is useful to determine the false positive in blood culture,too.
