T-SPOT.TB在结核感染诊断中的应用研究

【摘要】 目的评价结核感染T细胞斑点试验(T-SPOT.TB)对诊断结核的应用价值。方法对临床确诊的74例肺结核、17例肺外结核和88例健康体检者,进行外周血T-SPOT.TB、皮肤结核菌素试验(PPD)和萋-尼抗酸染色(AFB)的检测,并比较3种方法的诊断价值。结果 T-SPOT.TB检测对肺结核和肺外结核的灵敏度分别为91.89%、88.23%;T-SPOT.TB对结核的特异度为93.18%;PPD和AFB对肺结核的抗体灵敏度分别为70.00%、28.38%,对肺外结核的灵敏度分别为:41.18%、35.29%,特异度分别为95.45%、97.73%。T-SPOT.TB的灵敏度与二者相比,差异有统计学意义(P<0.01),特异度与二者相比,差异无统计学意义。结论酶联免疫斑点T-SPOT.TB实验检测结核敏感性强、特异性高,是快速诊断结核感染的有效手段。

【Abstract】 Objective To investigate the value of T cells sport test( T- SPOT. TB) in the diagnosis of tuberculosis infection.Methods Peripheral blood T- SPOT. TB,purified protein derivatives of tuberculin( PPD) skin test and acid- fast stain and tuberculosis antibody test( AFB) were measured in the peripheral blood from 74 patients with tuberculosis,17 patients with extrapulmonary tuberculosis and 88 healthy individuals,and the diagnostic value of 3 methods were compared. Results The sensitivity of T- SOPT. TB for the diagnosis of tuberculosis and extrapulmonary tuberculosis were 91. 89%,88. 23%; the specificity of T- SPOT. TB for tuberculosis was 93. 18%; While the sensitivity from tests of AFB and PPD for the diagnosis of active tuberculosis were 70. 00%,28. 38%,and the sensitivity to extrapulmonary tuberculosis were 41. 18%,35. 29%,and the specificity were 95. 45%,97. 73%. Comparing with the other two methods,sensitivity difference in T- SOPT. TB for diagnosing tuberculosis infection was statistically significant( P < 0. 01). While the difference in specificity was not statistically significant.Conclusion T- SOPT. TB assay is a sensitive and specific method for detecting Mycobacterium tuberculosis infection,which is better than PPD skin test and AFB,T- SOPT. TB has an important reference for diagnosing of tuberculosis quickly and accurately.
