宁波市子宫颈感染HPV型别分布和HPV16型E6、E7基因分析

【摘要】 目的了解宁波地区人乳头状瘤病毒(HPV)的型别分布,分析HPV16亚型病毒的癌相关基因E6、E7区基因变异情况。方法收集宁波市社区妇女的宫颈脱落细胞样本进行HPV分型检测,并对HPV16亚型株进行癌相关基因测序和序列分析。结果从778份样本检出HPV阳性215例,检出率为27.63%。其中检出HPV16型13株并成功获得其E6、E7基因序列。E6区发生5个位点的碱基颠换:G94A、T178G、T350G、C335T和A508C,其中T178G出现率最高,有9例,占69.23%。E7区有5个位点发生碱基颠换:A647G、G666A、T756G、T843C和T846C,其中A647G和T846C出现率最高,有9例,占69.23%。系统进化树分析显示,宁波地区HPV16型流行株存在亚洲变异型和欧洲变异型,未见非洲1型、2型、亚美型和北美型变异。结论宁波社区人群检出多种亚型的HPV病毒,大多数的感染者携带病毒的癌相关基因发生变异,存在潜在瘤变危险,应加强对人群感染状况和HPV流行株变异情况的监测,及时调整防治策略。

【Abstract】 Objective To investigate distribution of human papillomavirus( HPV) subtype and analysis of oncogene mutation among HPV16 type E6,E7 region in Ningbo. Methods The cervical exfoliated cells were collected from women in community in Ningbo and detected for HPV subtype. The oncogene of HPV16 type strains were sequenced. Results 215( 27. 63%) HPV positive samples were screened from 778 cases. 13 HPV16 type samples were sequenced with E6,E7 gene. 5 mutable points were base transversion in HPV16 E6 gene,which were G94 A,T178G,T350 G,C335T and A508 C,and T178 G was the most frequently seen with the mutation frequency of 69. 23%. 5 mutable points were base transversion in HPV16 E7 gene,which were A647 G,G666A,T756 G,T843C and T846 C,the A647 G and T846 C were the most frequently seen with the mutation frequency of 69. 23%. Analysis of the phylogenetic tree showed that only Asian and European variant subtype exist,but African type 1 and 2,Asian- American and North- American variants were not found. Conclusion Multiple HPV subtypes circulated in community population in Ningbo,and most oncogene of HPV strains were variated which may evolve into tumour wih high risk. The surveillance on population infection and variations of HPV epidemic strains should be strengthened. Meanwhile,a suitable prevention and control plot of HPV also needed implemented.
