【摘要】 目的:探讨干扰素α-2b(interferonα2b,IFNα2b)用于椎旁神经阻滞治疗急性期带状疱疹(herpes zoster,HZ)患者的临床疗效及对疱疹后神经痛(postherpetic neuralgia,PHN)的预防效果。方法:HZ患者60例,分为干扰素(A组)和对照组(B组);A组在椎旁神经阻滞药物配方中加用IFNα-2b,B组除药物配方中未用IFNα2b外,余治疗同A组。于治疗前及治疗后进行疼痛疗效评分、睡眠质量评分同时检测患者外周血中T淋巴细胞亚群及Ig G水平;统计治疗后4周和12周末PHN发生率;治疗结束后患者对治疗效果进行自我评价,并统计治疗次数。结果:A组患者治疗后疼痛疗效优于B组(P<0.001),治疗次数少于B组(P<0.001),患者自评结果优于B组(P<0.001),治疗后4周末和12周末PHN发生率均低于B组(P<0.05);两组治疗后睡眠质量均得到改善(P<0.001);两组患者治疗后各时间点与治疗前比较,外周血中CD3、CD4、CD4/CD8比值均升高,CD8水平降低,Ig G水平升高,且A组变化较B组显著(P<0.05)。结论:IFNα2b用于椎旁神经阻滞治疗HZ患者临床疗效确切,治疗后患者免疫功能可得到改善,能有效预防PHN的发生。更多还原

【Abstract】 Objective: To evaluate the clinical efficacy of paraspinal nerve blocking with interferon α-2b and the preventive efficacy of postherpetic neuralgia when treating acute herpes zoster. Methods: 60 cases of herpes zoster were randomly and evenly divided into interferon α-2b group(group A) and control group(group B). The only difference between group A and B was whether to use the interferon α-2b or not. For each patient, before and after treatment, we measured the T lymphocyte subsets and Ig G level in peripheral blood, and we recorded VAS pain score and sleep quality score as well. Then we calculated the incidence of post herpetic neuralgia at 4 and 12 weeks after treatment. At last, the patients made self-evaluation about the efficacy and we collect the treatment times. Results: After treatment, the pain relief of group A was more effective than group B(P < 0.001). The patients in group A had fewer therapy times(P < 0.001) and feel better than patients in group B after treatment(P < 0.001). The incidence of postherpetic neuralgia in group A was lower than that in group B at 4 and 12 weeks post treatment(P < 0.05). The quality of sleep was improved in both groups(P < 0.001). When compared with pretherapy, the levels of CD3, CD4, CD4/CD8 and Ig G in both groups were increased, and the CD8 level and VAS score were reduced. In addition, the changes were much more substantial in group A than in group B(P < 0.05). Conclusion: Paraspinal nerve block with interferon α-2b can treat herpes zoster patients, prevent the postherpetic neuralgia and improve patients’ immunity effectively.更多还原
