【摘要】 目的探讨提高外科重症监护室(SICU)护士对重症脑卒中患者早期肢体功能锻炼依从性的方法。方法 78例重症脑卒中患者,随机分为对照组35例(管理前)和质控组43例(管理后),通过小组管理、制订具体操作指南、组织严格培训、严格质量控制等综合措施对护理工作进行管理,提高依从性。6个月后对患者的肢体运动功能、认知能力和吞咽功能进行评价。结果质控组患者6个月后的Fugl-Meyer评定及简易智能精神状态检查量表(MMSE)评分提升明显优于对照组,差异有统计学意义(P<0.05)。6个月后,两组患者吞咽功能比较,差异无统计学意义(P>0.05)。结论采用专业化的综合管理措施可明显提高SICU护士对重症脑卒中患者早期肢体功能锻炼的依从性,改善患者的预后。更多还原

【Abstract】 Objective To investigate the method for improving compliance of nurse in stroke intensive care unit(SICU) in early functional exercise for severe stroke patients. Methods A total of 78 severe stroke patients were randomly divided into control group with 35 cases(before management) and quality-control group with 43 cases(after management). Management of nursing work was applied through comprehensive measures to improve compliance, such as group managing, formulating specific operational guide, organizing strict training, and strict quality control. After 6 months, patients’ movement function, cognitive competence, and swallowing function were evaluated. Results The quality-control group had much better improvements in Fugl-Meyer assessment and mini-mental state examination(MMSE) scores after 6 months than the control group, and the difference had statistical significance(P<0.05). There was no statistically significant difference of swallowing function between the two groups after 6 months(P>0.05). Conclusion The implement of professional comprehensive management measures can remarkably enhance the compliance of nurse in SICU in early functional exercise for severe stroke patients, and improve patients’ prognosis.更多还原
