【摘要】 目的探讨跟骨粉碎性骨折患者采用切开复位植骨结合钢板内固定治疗的临床疗效。方法跟骨粉碎性骨折患者63例,采用切开复位植骨并结合钢板内固定治疗方案,术后比较跟骨粉碎性骨折中不同骨折类型患者的恢复效果及记录有无合并并发症的存在,并对结果进行讨论分析。结果63例患者均获得随访,Ⅱ型及Ⅲ型的预后良好程度均大于Ⅳ型(P<0.05)。Ⅱ型相比较Ⅲ、Ⅳ型术后并发症的发生率更低(P<0.05)。所有患者术后与术前Gissane夹角、Bohler角比较差异有统计学意义(P<0.05)。结论应用切开复位植骨结合钢板内固定治疗跟骨粉碎性骨折其临床恢复效果与骨折类型有直接关系,Ⅱ型骨折最适合,而Ⅲ型和Ⅳ型骨折相对适应。整体效果良好,值得推广。更多还原

【Abstract】 Objective To explore the clinical effect of open reduction implantation combined with steel plate internal fixation in the treatment of calcaneal comminuted fracture. Methods Combination treatment by open reduction implantation and steel plate internal fixation was applied for 63 patients with calcaneal comminuted fracture. Comparisons were made on the recovery effect between those patients and other patients with different types of calcaneal comminuted fracture. The complications were recorded, and the research results were analyzed. Results Follow-up was carried on for all the 63 cases. The prognosis of type Ⅱ and Ⅲ were better than that of type Ⅳ(P<0.05). The incidence of complications was lower in type Ⅱ than in type Ⅲ and type Ⅳ(P<0.05). There were statistically significant differences of Gissane angle and Bohler angle before and after treatment in all the cases(P<0.05). Conclusion The application of open reduction implantation combined with steel plate internal fixation in the treatment of calcaneal comminuted fracture has varied clinical recovery effect, which is related to different fracture types. Type Ⅱ is the most suitable one, while type Ⅲ and type Ⅳ are comparably suitable. This method has overall good effect, and is worth promotion.更多还原
