【摘要】 目的:探讨单向式全胸腔镜肺叶切除术在Ⅰ/Ⅱ期非小细胞肺癌的适应证、手术操作要点及疗效。方法:回顾分析2011年9月~2014年9月秦皇岛市第一医院胸外科在胸腔镜下行单向式肺叶切除术的患者218例,其中男141例,女77例;年龄46~75岁,平均61岁。观察术中淋巴结清扫数、手术时间、术中出血量、术后胸腔引流时间、术后住院时间和并发症发生率。结果:全部218例患者手术过程顺利,手术时间94.21±7.27min,术中出血234.01±112.57ml,术后胸腔引流9.02±3.06d。出现漏气6例,乳糜胸4例,声音嘶哑4例。原发性肺癌患者清扫纵隔淋巴结数目7.78±0.89枚,住院时间11.4±3.28d;无二次手术及死亡病例。结论:单向式全胸腔镜肺叶切除术创伤小、恢复快、疼痛轻,手术安全性、根治性与传统开胸手术相似,是治疗早期非小细胞肺癌的有效手术方式。更多还原

【Abstract】 Objective:To summarize the experience of single-direction video-assisted thoracoscopic pulmonary lobectomy in early-stage non-small cell lung cancer(NSCLC)Methods:Two-hundred and eighteen patients(141 males,77 females,age 46-75 years old)in our hospital with NSCLC underwent single-direction video-assisted thoracoscopic pulmonary lobectomy from Sep.2012 to Sep.2014 were analyzed.The number of resected lymph nodes,operation time,intraoperative blood loss,postoperative thoracic drainage,hospitalization time after operation and postoperative complications were observed.Results:All operations in 218 patients were successfully accomplished.The operation time was 94.21 ±7.27 min.The amount of intraoperative bleeding was 234.01 ±112.57 ml.The average postoperative thoracic drainage time was 9.02 ±3.06 days.There were 6 patients with air leaks,6 patients with chyloplura and 4 with hoarseness.The numbers of lymph nodes resected were 7.78±0.89.The average hospitalization time was 11.4 ±3.28 days.No patient needed a second operation,mortality was nil.Conclusion:The pulmonary lobectomy by single-direction video-assisted thoracoscopy is a safe and effective method for early-stage NSCLC.The advantages were rapid postoperative recovery,less pain and trauma.更多还原
