【摘要】 目的探讨后腹腔镜巨大肾积水无功能肾切除的手术技巧。方法回顾性分析2010年9月至2014年5月采用后腹腔镜切除巨大肾积水无功能肾48例的临床资料。其中男33例,女15例,平均44岁;巨大肾积水的病因包括输尿管结石38例、先天性肾盂输尿管交界处狭窄8例,异位血管压迫引起2例。观察手术时间、术中出血量、住院天数及手术效果。结果 48例手术均获成功,无中转开放手术。术中术后均未出现明显并发症;手术时间40~180 min,平均90 min,术中出血量20~180 ml,平均60 ml,术后住院时间5~8 d。结论后腹腔镜下巨大肾积水的无功能肾切除安全可行,通过一定的手术技巧有助于术中手术操作。更多还原

【Abstract】 Objective To investigate the skill of retroperitoneal laparoscopic nephrectomy for giant hydronephrosis. Methods From Sep. 2010 to May 2014,48 patients( 33 males and 15 females) with giant hydronephrosis were carried out retroperitoneal laparoscopic nephrectomy. The mean age was 44 years. The pathogenesis of giant hydronephrosis were various,including 38 cases of ureteral stones,eight cases of congenital ureteroplvic junction obstruction and 2 cases of ectopic vessel. The operative time,blood loss,hospital stay and the efficiency were recorded. Results All operation were performed successfully and no case was converted to open surgery. There was no serious complication. The mean operative time was90 min,ranging from 40 to 180 min. The mean blood loss were 60 m L,ranging from 20 to 180 m L. The postoperative hospital stay was 5 to 8 days. Conclusions The retroperitoneal laparoscopic nephrectomy is safe and feasible for giant hydronephrosis.
