切开复位内固定治疗成人肱骨小头骨折伴或不伴后髁骨折临床效果观察
CLINICAL OBSERVATION OF FRACTURE OF CONDYLE WITH OR WITHOUT POST BY OPEN REDUCTION AND INTERNAL FIXATION IN ADULT HUMERAL HEAD FRACTURES
【摘要】 目的探讨切开复位内固定治疗成人肱骨小头骨折伴或不伴后髁骨折临床效果。方法选取2011年2月—2014年12月该院收治的30例成人肱骨小头骨折伴或不伴后髁骨折患者,所有患者均给予切开复位内固定治疗,观察患者手术治疗后肘关节伸屈活动度、前臂旋转活动度、肘关节外翻稳定性、Mayo肘关节功能评分(MEPS)等。结果骨折侧肘关节屈曲活动度、屈伸活动度明显大于健侧肘关节,伸直活动度小于健侧,差异均有统计学意义(P<0.05),骨折侧肘关节与健侧肘关节前臂旋转活动度比较差异无统计学意义(P>0.05);肱骨小头骨不伴有后髁骨折患者肘关节屈伸活动度大于肱骨小头骨伴有后髁骨折患者,差异有统计学意义(P<0.05),肱骨小头骨不伴有后髁骨折患者MEPS评分大于肱骨小头骨伴有后髁骨折患者,差异有统计学意义(P<0.05)。结论切开复位内固定可有效改善成人肱骨小头骨折伴或不伴后髁骨折临床效果,患者若伴有后髁骨折则需加强治疗,巩固治疗效果。
【Abstract】 Objective To investigate the clinical effect of open reduction and internal fixation in adult humeral head fractures with or without posterior condylar fracture.Methods From February 2011 to December 2014 in author’s hospital,a total of 30 cases of adult humeral head fractures with or without posterior condylar fracture patients were collected,all patients were given open reduction and internal fixation,and some functions,such as the elbow extension flexion movement,rotational movement of the forearm,elbow valgus stability of the festival,and the Mayo Elbow Performance Score(MEPs),were observed after surgical treatment.Results Fractures of the lateral elbow flexion,flexion and extension activity was significantly greater than that of the contralateral elbow extension,whose activities was lower than that of the healthy side,the differences were statistically significant(P<0.05).Fracture of elbow joint and the contralateral elbow forearm rotation activity differences had no statistical significance(P>0.05).Humeral capitellum was not associated with posterior condylar fractures of the elbow joint flexion and extension,its activities was greater than the humerus small skull associated with posterior condylar fracture patients,the difference was statistically significant(P<0.05).Humeral capitellum were not associated with posterior condylar fracture patients with scores greater than the humerus small skull associated with posterior condylar fracture patients,the difference was statistically significant(P<0.05).Conclusion Open reduction and internal fixed effectively improve the capitulum humeral fracture with or without posterior condylar fractures,patients if accompanied by posterior condylar fracture need strengthening treatment for consolidating curative effect.
