腰椎单侧与双侧椎弓根螺钉固定的生物力学对比性研究
Biomechanical comparative study of lumbar unilateral and bilateral pedicle screw fixation
【摘要】 目的本研究采用小牛腰椎标本进行单侧和双侧椎弓根螺钉固定,测试并比较两者的生物力学稳定性,以探讨单侧椎弓根钉固定多节段(包括双节段和三节段)腰椎的可行性。方法采用6具小牛体新鲜标本制成相应的实验测试模型进行生物力学测试,依次记录不同标本在不同处理状态下各节段的运动范围,所得数据进行统计学分析。结果单节段单侧固定与双侧固定在前屈/后伸、左/右侧弯、左/右旋6个方向上的生物力学稳定性无显著性差异(P>0.05)。多节段(包括双节段和三节段)的单侧固定除右侧弯外,在前屈/后伸、左侧弯、左/右旋方向上与双侧固定对比无显著性差异(P>0.05)。结论单侧腰椎椎弓根螺钉固定多节段(包括双节段和三节段)的生物力学性能良好,刚度适中,腰椎可获得可靠的稳定性。
【Abstract】 Objective To investigate the feasibility of the unilateral pedicle screw fixation in multi-segment(including double segments and three segments) lumbar spine via biomechanical comparison of unilateral and bilateral pedicle screw fixation of calf lumbar spine. Methods The range of motion in each fresh calf specimen(L1-S1) was nondestructively tested in flexion-extension, left-right lateral bending and left-right axial rotation by a spine testing machine. The data obtained were analyzed via SPSS Version 16.0. Results There was no significant difference of the ROM at single segment unilateral and bilateral fixation in flexion/extension, left lateral bending and left/right axial rotation(P>0.05). Apart from the ROM of right lateral bending, there was no significant difference of the ROM at multi-segment(including double and three segments) unilateral and bilateral fixation in flexion/extension, left lateral bending and left/right axial rotation(P>0.05). Conclusion There was moderate stiffness and reliable stability in unilateral pedicle screw fixation of multi-segment(including double and three segments).
